超声对乙肝肝硬化背景小肝癌的诊断研究

摘　　要：目的:探讨超声对乙肝肝硬化背景小肝癌的诊断价值。方法:收集2016年4月-2018年4月本院收治的乙肝肝硬化背景小肝癌患者80例,对其依次采用超声与CT诊断检查,比较两者的诊断能力。患者经过射频消融治疗后,分别采取超声与CT检查对疗效进行评估,采取比较两者的一致性。结果:超声对≤1 cm的小肝癌诊断能力明显比增强CT弱(P<0.05),对于1 cm<肿瘤直径≤2 cm的小肝癌超声诊断与增强CT诊断比较,差异无统计学意义(P>0.05);治疗后1、3个月时,超声与增强CT对治疗有效率评价结果比较,差异均无统计学意义(P>0.05)。结论:与CT检查相比,超声对直径小于1 cm的小肝癌诊断能力较差,但对1 cm<肿瘤直径≤2 cm的小肝癌诊断能力较好,经济便捷,且无辐射,安全,并能对射频消融疗效评估的价值较高。Objective:To explore the diagnostic value of ultrasound in small hepatocellular carcinoma with hepatitis B cirrhosis.Method:From April 2016 to April 2018,80 cases of small hepatocellular carcinoma with background of hepatitis B cirrhosis were collected in our hospital.Ultrasound and CT were used in order to compare their diagnostic ability.After radiofrequency ablation,the patients were assessed by ultrasound and CT,and their consistency was compared.Result:The diagnostic ability of ultrasound for small hepatocellular carcinoma(<1 cm)was significantly weaker than that of contrast-enhanced CT(P<0.05),and there was no significant difference between ultrasound and contrast-enhanced CT for small hepatocellular carcinoma(1 cm<diameter≤2 cm,P>0.05).At 1 and 3 months after treatment,there was no significant difference between ultrasound and contrast-enhanced CT(P>0.05).Conclusion:Compared with CT,ultrasound has a poor diagnostic ability for small hepatocellular carcinoma with a diameter of less than 1 cm,but it has a better diagnostic ability for small hepatocellular carcinoma with a diameter of less than 2 cm.It is economical,convenient,non-radiation,safe and has a higher value in evaluating the efficacy of radiofrequency ablation.
