心肺复苏持续胸外心脏按压时呼吸机分钟通气量与气道高压报警值设置的研究

摘　　要：目的:探讨呼吸机分钟通气量和气道高压报警值的设置值对心肺复苏持续胸外按压效果的影响。方法:收治呼吸心搏骤停需行心肺复苏患者86例,随机分为对照组和观察组,每组各43例。对照组采用常规潮气量及气道高压报警设置,观察组根据患者具体情况满足通气,适当增加分钟通气量目标值及提高气道高压报警设置,比较两组治疗效果。结果:心肺复苏10 min后,观察组p H值、血氧分压(PaO2)、血二氧化碳分压(PaCO2)、HCO3-、血氧饱和度(SaO2)及血乳酸均优于对照组(P<0.05);观察组心肺复苏成功率高于对照组(P<0.05);观察组吸气峰压(PIP)低于对照组(P<0.05)。结论:为接受心外复苏持续胸外心脏按压的患者提高呼吸机分钟通气量,上调气道高压报警值,治疗效果较好,安全性高。Objective:To explore the influence of set value of minute ventilation and respirator high pressure alarm value of ventilator on the effect of cardiopulmonary resuscitation with persistent external cardiac compression.Methods:86 patients with respiratory and cardiac arrest and cardiopulmonary resuscitation were selected,they were randomly divided into the control group and the observation group,the control group was given conventional tidal volume and airway high pressure alarm settings,in the observation group,we appropriately increased the target value of minute ventilation volume and increased the airway high pressure alarm setting according to the specific conditions of patients,we compared the therapeutic effects of the two groups.Results:After 10 minutes of cardiopulmonary resuscitation,in the observation group,pH,PaO2,PaCO2,HCO3-,SaO2 and blood lactic acid were all better than those of the control group(P<0.05).The success rate of cardiopulmonary resuscitation in the observation group was higher than that in the control group(P<0.05).The PIP of the observation group was lower than that of the control group(P<0.05).Conclusion:The effect of improving minute ventilation and respirator high pressure alarm value of ventilator for patients with cardiopulmonary resuscitation with persistent external cardiac compression was good,and the safety was high.
