Vitapex根管内置药联合盐酸米诺环素对慢性牙周炎的临床疗效

摘　　要：目的:探讨Vitapex糊剂根管内置药联合盐酸米诺环素对慢性牙周炎的临床疗效。方法:选取2016年3月—2017年12月就诊于我院口腔科门诊明确诊断慢性牙周炎的患者120例,随机分成对照组(氧化锌丁香油糊剂根管治疗,n=60)与观察组(Vitapex糊剂根管治疗联合牙周袋内注入盐酸米诺环素,n=60),均治疗6周,对比两组患者治疗前、后视觉模拟评分(VAS)、相关炎症指标(高敏C反应蛋白、白介素-1β、白介素-2、白介素-17)、牙周指标(菌斑指数、牙龈指数、牙周袋探诊深度、出血指数)、总有效率的异同。结果:(1)与治疗前比较,治疗后6周后两组患者牙周相关指标、VAS评分、炎症指标均明显降低,观察组明显优于对照组(均P <0. 05);(2)对照组总体有效率为78. 3%,观察组为95. 0%,两组比较差异有统计学意义(P=0. 004)。结论:Vitapex糊剂根管内置药联合牙周袋内注入盐酸米诺环素可显著改善疼痛症状与炎症反应,降低牙周相关指标,显著提高总体有效率,值得临床推广。Objective:To observe the clinical efficacy of Vitapex root canal therapy combined with minocycline hydrochloride in the treatment of chronic periodontitis.Methods:120 patients with chronic periodontitis who were diagnosed in the department of stomatology of our hospital from March 2016 to December 2017 were selected for the study.there were randomly divided into the control group(n=60)and the observation group(n=60).Patients in the control group were given conventional root canal therapy(Zinc oxide clove oil pasteroot canal therapy),while patients in the observation group were given Vitapex root canal therapy combined with minocycline hydrochloride.Both group were treated 6 weeks.The differences of visual analogue scale(VAS),the related inflammation index as[high sensitive C reactive protein(hs-CRP),interleukin-1β(IL-1β),interleukin-2(IL-2)and interleukin-17(IL-17)],the periodontal related indexes as[plaque index(PLI),gingival index(GI),probing depth(PD)and bleeding index(BI)]and the clinical overall efficiency between the two groups were compared.Results:(1)Compared with the pre-treatment,the levels of VAS,the related inflammation index as hs-CRP,IL-1β,IL-2 and IL-17,the periodontal related indexes as PLI,GI,PD and BI in the two groups were significantly decreased(all P<0.05),but the levels in the observation group were signifcantly lower than those in the control group(all P<0.05).(2)The clinical overall efficiency of the observation group was 95.0%,which was significantly higher than that of the control group(78.3%),the difference was statistically significant(P=0.004).Conclusion:The Vitapex root canal therapy combined with minocycline hydrochloride can improve the pain symptoms and inflammatory response of the patients with chronic periodontitis,reduce the periodontal related indexes,and improve the clinical overall efficiency,which is worthy of clinical promotion.
