晶状体溶解性青光眼的临床处理效果

摘　　要：目的探讨白内障过熟期所致晶状体溶解性青光眼的临床处理策略和疗效。方法对17例(17眼)晶状体溶解性青光眼患者临床资料进行分析,手术方式包括小切口非超声乳化白内障摘除术或联合人工晶状体植入术,白内障超声乳化术或联合人工晶状体植入术。术后随访3个月观察手术效果。结果 17眼均顺利完成手术。术后1个月11眼视力不同程度提高。6眼过高的眼压致视神经损害,视力无提高。术后16眼的眼压均恢复正常,1眼术后眼压为30~42 mm Hg,经降眼压药物治疗效果不佳,后行Ex-press引流钉植入术控制眼压。结论白内障摘除为治疗晶状体溶解性青光眼的有效方式,是否植入人工晶状体取决于术中评估,开展白内障防盲教育与筛查是预防本病的重要手段。Objective To explore the clinical management strategy and curative effect of phacolytic glaucoma caused by hypermature stage of cataract.Methods The clinical data of 17 patients(17 eyes)with phacolytic glaucoma were analyzed.These patients were treated with small incision non-phacoemulsification cataract extraction or combined with intraocular lens implantation as well as phacoemulsification or combined with intraocular lens implantation.All cases were followed up for 3 months to observe operation effect.Results 17 eyes successfully completed the operation surgery.Visual acuity improved in 11 eyes with varying degrees 1 month after operation,while 6 eyes with optic nerve lesion cased by high intraocular pressure(IOP)had no improvement of visual acuity.The IOP of 16 eyes returned to normal after operation,the IOP of 1 eye was 30~42 mmHg,the effect was poor by IOP-lowering drugs,and the IOP was controlled by Ex-press drainage nail implantation.Conclusion Cataract extraction is an effective method to treat phacolytic glaucoma.Intraocular lens implantation is decided by surgery evaluation.Education in preventing blindness and cataract screening for people are important means to prevent this disease.
