舍曲林合并不同剂量丁螺环酮对老年抑郁症患者临床疗效和安全性的影响

摘　　要：目的探讨舍曲林合并常规剂量及大剂量丁螺环酮对老年抑郁症患者快速起效率、负面情绪评分及不良反应发生率的影响。方法研究对象选取我院2014年5月至2016年5月收治的老年抑郁症患者共150例,按随机数字表法分为低剂量组(75例)和高剂量组(75例),分别在服用舍曲林的基础上加用常规剂量(15mg/d)和大剂量(40mg/d)丁螺环酮。比较两组患者治疗6周后疗效,治疗前后临床疗效总评量表(CGI)评分,汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评分及不良反应发生率等。结果高剂量组患者近期疗效、快速起效率显著优于低剂量组,差异有统计学意义(P<0.05)。高剂量组患者治疗后CGI评分、HAMA及HAMD评分均显著低于低剂量组,且治疗后两组各评分较治疗前均降低,差异有统计学意义(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论舍曲林合并大剂量丁螺环酮可有效缓解老年抑郁症患者临床症状体征,缩短起效时间,改善负面情绪状态,且并未增加不良反应发生几率。Objective To discuss the effects of sertraline combined with routine and large doses of buspirone on the rapid-onset rate,negative emotion score and adverse reactions incidence in elderly patients with depression.Methods A total of 150 elderly patients with depression admitted to our hospital from May 2014 to May 2016 were selected as study objects and were divided into low-dose group(75 cases)and high-dose group(75 cases)according to the random number table which the routine dose(15mg/d)and large dose(40mg/d)of buspirone were added on the basis of sertraline respectively.The curative effect after 6 weeks,the clinical global impression(CGI)score before and after treatment,the Hamilton anxiety scale(HAMA)and the Hamilton depression scale(HAMD)score,and adverse reactions incidence of two groups were compared.Results The short-term effects and rapid-onset rate of patients in the high-dose group were significantly better than those in the low-dose group,and the differences were statistically significant(P<0.05).The CGI,HAMA and HAMD scores of the patients after treatment in the high-dose group were significantly lower than those in the low-dose group,and the scores of the two groups were significantly lower after treatment than before treatment,with statistically significant differences(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Sertraline combined with large dose of buspirone can effectively alleviate the clinical symptoms and signs of elderly patients with depression,shorten the onset time,and improve the negative emotional state without increasing the incidence of adverse reactions.
