CDU联合UE、Fine-Flow评估小乳癌术前新辅助化疗的效果研究

摘　　要：目的研究彩色多普勒超声(CDU)联合超声弹性成像(UE)、超声高精细血流成像技术(Fine-Flow)在评估小乳癌术前新辅助化疗(NAC)中的应用价值。方法选取行NAC的74例小乳癌患者为研究对象,分别采用CDU、CDU联合UE、Fine-Flow检查,评价近期疗效、远期疗效并对切除病灶进行病理分析。对比分析NAC前后肿块大小、血流速度、阻力指数值、UE评分变化。比较不同检查方式评估NAC疗效的灵敏度、特异度、符合率。结果NAC后完全缓解60例,部分缓解6例,无变化6例,进展2例,治疗总有效率为89. 19%。NAC后对小乳癌患者进行3年随访,1、3年生存率分别为86. 49%、81. 08%。NAC后病理分析显示,肿块病理分级Ⅰ~Ⅲ级48例,占64. 86%,Ⅳ~Ⅴ级26例,占35. 14%。NAC后肿块大小较NAC前明显减小,血流速度、阻力指数值及UE评分较NAC前明显降低,差异有统计学意义(P <0. 05)。74例小乳癌患者病理评估NAC有效59例,无效15例。CDU+UE+Fine-Flow评估小乳癌NAC疗效的灵敏度、特异度、符合率较单独CDU明显提高,差异有统计学意义(P <0. 05)。结论 CDU联合UE、Fine-Flow技术能够准确判断小乳癌NAC疗效,灵敏度、特异度、符合率较单独CDU更高,具有良好的应用前景。Objective To investigate the clinical application value of color Doppler ultrasonography(CDU)combined with magnetic resonance imaging(UE)and Fine-Flow ultrasonography(Fine-Flow)in evaluating the therapeutic effects of neoadjuvant chemotherapy(NAC)on small breast cancer.Methods A total of 74 patients with small breast cancer who were admitted and treated in our hospital from January 2015 to January 2018 were enrolled in the study.All the patients were treated by NAC and were followed up for 3 years after NAC,then the short-term and long-term therapeutic effects were evaluated by simple CDU and CDU combined with UE and Fine-Flow.Moreover the changes of tumor size,blood flow velocity,resistance index and UE scores before and after NAC were observed and compared.The sensitivity,specificity and coincidence rate of different examination methods were compared.Results After NAC,60 cases were complete remission,6 cases were partial remission,6 cases were not change and 2 case were progression,thetotal effective rate was 89.19%.After 3-year follow up,the 1-year survival rate and 3-year survival rate were 86.49%and 81.08%,respectively.Pathological analysis after NAC showed that 48 cases were at gradeⅠ~Ⅲ,accounting for 64.86%,and 26 cases were at gradeⅣ~Ⅴ,accounting for 35.14%.After NAC,tumor size,blood flow velocity,resistance index and UE scores were significantly decreased,as compared with those before NAC(P<0.05).According to the pathology classification criteria,59 cases were effective and 15 cases were ineffective.The sensitivity,specificity and coincidence rate of CDU+UE+Fine-Flow in evaluating the therapeutic effects of NAC on small breast cancer were significantly higher than those by simple CDU examination(P<0.05).Conclusion CDU combine UE and Fine-Flow can accurately assess the therapeutic effects of NAC on small breast cancer,moreover the sensitivity,specificity and coincidence rate of CDU+UE+Fine-Flow in evaluating the therapeutic effects of NAC on small breast cancer are significantly higher than those
