孕激素保守疗法与刮宫手术治疗早期子宫内膜癌的临床效果对比

摘　　要：目的对比分析孕激素保守疗法与刮宫手术治疗早期子宫内膜癌的临床效果。方法将76例早期子宫内膜癌患者随机分为2组,每组38例。对照组给予常规的刮宫手术治疗,观察组给予孕激素保守疗法治疗。比较2组的临床治疗效果;分别于治疗前后采用免疫组织化学法检测2组的孕激素受体以及雌激素受体,并随访1年,观察2组患者的妊娠成功率和复发情况。结果观察组的有效率为92. 10%(35/38),明显高于对照组的73. 68%(28/38)(P <0. 05)。2组治疗后的孕激素受体以及雌激素受体评分均明显降低(P <0. 05),且观察组的降低幅度更大(P <0. 05);观察组的妊娠成功率为31. 58%(12/38),明显高于对照组的13. 16%(5/38)(P <0. 05)。对照组的复发率为23. 68%(9/38),明显高于观察组的7. 89%(3/38)(P <0. 05)。结论孕激素保守疗法治疗早期子宫内膜癌的临床效果明显优于刮宫手术,可以有效改善患者的激素水平,提高妊娠成功率,且降低复发率。Objective To comparethe clinical efficacy between progesterone conservative therapy and dilatation and curettage in the treatment of early endometrial cancer.Methods Selected 76 cases of patients with early endometrial cancer who were treated and divided into 2 groups randomly.The control group was given routine curettage,and the observation group received conservative treatment with progesterone.The clinical treatment effects were compared between the 2 groups,respectively before and after treatment were detected progesterone receptor and estrogen receptor,and followed up for 1 years,to observe the success rate of pregnancy and recurrence of the 2 groups of patients.Results After treatment,the effective rate of the observation group was 92.10%(35/38),significantly higher than that of the control group 73.68%(28/38)(P<0.05);The progesterone receptor and estrogen receptor score were significantly lower in the 2 groups after treatment(P<0.05),and the decrease was greater in the observation group(P<0.05);The pregnancy success rate of the observation group was 31.58%(12/38),which was significantly higher than that of the control group 13.16%(5/38)(P<0.05);The recurrence rate of the observation group was 23.68%(9/38),which was significantly lower than that of the control group 7.89%(3/38)(P<0.05).Conclusion Progesterone conservative treatment of early endometrial cancer is better than curettage,can effectively improve the patient's hormone level,improve the pregnancy success rate,and reduce the recurrence rate.
