摘　　要：目的探讨骨桥蛋白(osteopontin,OPN)与喉鳞状细胞癌(laryngeal squamous cell carcinoma,LSCC)发生、侵袭转移的关系及临床意义。方法选用Pubmed、Web of science、Embase、Cochrane Library、维普、中国知网及万方等数据库检索2005～2017年国内外发表的OPN表达与喉癌发生、侵袭转移相关的病例对照研究文献,应用RevMan5.3统计软件对各研究原始数据进行统计分析。结果共检索到相关文献113篇,纳入其中符合要求的文献15篇,喉癌组787例患者中检出OPN阳性者558例(70.90%,558/787),正常对照组453例中检出OPN阳性者84例(18.54%,84/453),喉癌组OPN检出率明显高于对照组;Meta分析显示,在喉癌组中,OPN的表达与肿瘤的发生(OR=12.44,95%CI为9.13～16.95)、淋巴结转移(OR=4.92,95%CI为3.32～7.31)、组织分化程度(OR=3.39,95%CI为2.37～4.86)、临床分期(OR=4.67,95%CI为3.21～6.79)显著相关;而与性别(OR=1.01,95%CI为0.57～1.77)、年龄(OR=0.93,95%CI为0.61～1.43)、肿瘤部位(OR=1.48,95%CI为0.93～2.38)无关。结论 OPN的表达与喉癌的发生、发展密切相关,可作为判断喉鳞状细胞癌侵袭转移的指标。Objective To investigate the relationship between osteopontin(OPN)and the occurrence,invasion and metastasis of laryngeal squamous cell carcinoma(LSCC)and clinical significance.Methods The cases of OPN and LSCC were selected from PubMed,Web of science,Embase,Cochrane Library,CNKI,VIP,Wanfang database and Chinese HowNet(2005~2017).The statistical analysis of the original data was carried out using RevMan 5.3 statistical software.Results The Meta analysis results showed that inn laryngeal squamous cell carcinoma,OPN expression was significantly correlated with tumorigenesis(OR=12.44,95%CI=9.13~16.95),lymph node metastasis(OR=4.92,95%CI=3.32~7.31),the degree of differentiation(OR=3.39,95%CI=2.37~4.86),and clinical stages(OR=4.67,95%CI=3.21~6.79).However,it was not related to sex(OR=1.01,95%CI=0.57~1.77),ages(OR=0.93,95%CI=0.61~1.43),and tumor sites(OR=1.48,95%CI=0.93~2.38).Conclusion The expression of OPN is closely related to the occurrence and development of laryngeal squamous cell carcinoma.It can be used as an indicator of the invasion and metastasis of laryngeal squamous cell carcinoma.
