彩色多普勒超声联合超声弹性成像和超声高精细血流成像技术对小乳癌的诊断价值

摘　　要：目的研究彩色多普勒超声(CDU)联合超声弹性成像(UE)、超声高精细血流成像技术(Fine-Flow)诊断小乳癌的临床价值。方法选取2015年1月至2018年1月收治的110例乳腺小肿块患者为研究对象(共120个乳腺病灶),分别采用CDU、CDU联合UE、Fine-Flow检查,判断小肿块良、恶性,并与病理检查结果进行比较。比较不同检查方式的灵敏度、特异度、符合率、血流分级情况、诊断小乳癌的敏感性、漏诊及误诊情况。结果病理检查结果显示,110例患者共120个乳腺小肿块中恶性74个,良性46个。CDU+UE+Fine-Flow诊断乳腺小肿块的灵敏度、特异度、符合率、阳性预测值、阴性预测值较单独CDU明显提高,差异有统计学意义(P <0. 05)。CDU+UE+Fine-Flow诊断最大径<1 cm、1～2 cm的小乳癌病灶的灵敏度较单独CDU明显提高,漏诊率和误诊率较单独CDU明显降低,差异有统计学意义(P <0. 05)。结论 CDU联合UE、Fine-Flow技术判断乳腺小肿块良、恶性与病理检查结果具有良好的一致性,灵敏度、特异度、符合率较单独CDU更高,尤其能够有效提高小乳癌的诊断率,具有良好的应用前景。Objective To investigate the diagnostic value of color doppler ultrasonography(CDU),magnetic resonance imaging(UE)and fine-flow ultrasonography(Fine-Flow)in small breast cancer.Methods A total of 110 cases of breast tumor(120 lesions)were collected from January 2015 to January 2018,which were detected by CDU,CDU+UE+Fine-Flow.Ultrasonographic features were analyzed in the two examination methods to judge whether the small lumps were benign or malignant,moreover,the examination results were compared with the pathological examination results after surgery.The consistency,the coincidence rate,sensitivity,specificity,blood flow classification,diagnosis rate,missed diagnosis rate and misdiagnosis rate were observed and compared between the two examination methods.Results The pathological examination results showed that among the 110 patients(120 small breast lumps),74 cases were malignant and 46 cases were benign.The sensitivity,specificity and accuracy of CDU+UE+Fine-Flow in diagnosis of small breast lumps were significantly higher than those by single CDU examination(P<0.05).The sensitivity of CDU+UE+Fine-Flow in diagnosis of small breast cancer(≤1cm,1~2cm)was significantly higher than that by single CDU examination,moreover,missed diagnosis rate and misdiagnosis rate were in diagnosis of small breast cancer(≤1cm,1~2cm)were significantly lower than those by single CDU exaination(P<0.05).Conclusion The diagnostic results by CDUcombined with UE,Fine-Flow in diagnosis of small breast cancer are good coincident with pathological results,moreover,the sensitivity,specificity and coincidence of combination detection are higher than those by single CDU examination,especially,combination detection can improve the diagnosis rate of minimal breast cancer,thus,which has good clinical application prospect.
