摘 要:目的观察基于"岗位胜任力"的螺旋式上升轮转计划在心内科住培中的应用效果。方法选取漯河市中心医院2014级住培学员作为对照组,共52人; 2015级住培学员设为观察组,共53人。对照组常规在心内科轮转4个月,执行普通三级医师查房制度;观察组采用基于"岗位胜任力"的螺旋式上升的轮转计划,分2次完成心内科住培计划,在住培第1年分别于心内科和心电图室各轮转1个月,在住培第2、3年,再于心内科轮转2个月,第2次在心内科轮转时实行以提高临床思维能力为核心的教学查房制度,包括独立的预查房和问题导向的正式查房2部分。心内科轮转结束时,要求两组住培学员填写调查问卷,评估自己的专业态度及在问诊、体格检查、临床判断、沟通、应用临床知识、团队协作、决策、理解并运用所学知识、综合分析、师生间沟通、健康教育方面的能力,根据问卷调查结果进行对比分析。结果观察组住培医师的专业态度及在问诊、体格检查、临床判断、沟通、应用临床知识、团队协作、决策、理解并运用所学知识、综合分析、师生间沟通、健康教育方面的能力评价均优于对照组,差异有统计学意义(均P <0. 05)。结论基于"岗位胜任力"的螺旋式上升的轮转计划可有效提高住培医师综合能力,值得在住院医师规范化培训中推广应用。Objective To observe the application effect of the spiral-rise rotation plan based on“position competency”in the standardized training of residents in department of cardiology.Methods Fifty-two standardized training residents in Luohe Central Hospital were selected as the control group,and 53 standardized training residents were placed in the observation group.The residents in control group were routinely trained in cardiology department for 4 months,performing normal three-level physician ward round system.The observation group performed spiral-rise rotation plan based on“position competency”and completed the cardiology residential training program in two sessions.In the first year of residential training,the students in observation group were trained in cardiology department and electrocardiogram room for 1 month respectively.In the second and the third year of residential training,they were trained for another 2 months in cardiology department.In the second session of the cardiology residential training,the teaching ward round system with the core of improving clinical thinking ability was implemented,including the independent preview ward round and the problem-oriented formal ward round.At the end of the training in department of cardiology,2 groups of students were required to fill out a questionnaire to assess their professional attitude and abilities in consultation,physical examination,clinical judgment,communication,application of clinical knowledge,teamwork,decision-making,understanding and application of knowledge,comprehensive analysis,inter-teacher-to-student communication.Answers of questionnaire of the two groups were compared and analyzed.Results The professional attitude and abilities in consultation,physical examination,clinical judgment,communication,application of clinical knowledge,teamwork,decision-making,understanding and application of knowledge,comprehensive analysis,communication between teachers and students,health education of residents in observer group were all superior to co
