摘　　要：目的:研究不同植入扭矩对牙种植体-骨结合的影响。方法:采用拉丁方实验设计及完全随机设计相结合的嵌套实验方法。4只beagle犬分别选取4个下颌前磨牙区植入位点,每个位点植入两枚BLBⅢ种植体,进行植入扭矩≤15 N·cm(A组)、25 N·cm(B组)、35 N·cm(C组)、≥45 N·cm(D组) 4组实验。32枚种植体根据植入时间点不同随机分为Ⅰ组(拔牙后即刻种植,8个位点)和Ⅱ组(Ⅰ组种植完成后1月种植,8个位点)。2个月时处死动物,处死前行四环素-茜素红双色荧光标记。标本进行X线检查、光学及荧光显微镜观察、定量组织学分析。结果:32枚种植体均无松动脱落,在Ⅰ、Ⅱ组中,B、C组的骨平均矿化率、种植体-骨结合率、骨结合指数均高于A、D组(P <0. 01)。结论:植入扭矩25 N·cm、35 N·cm较植入扭矩≤15 N·cm、≥45 N·cm更利于BLBⅢ牙种植体-骨结合。Objective:To investigate the effects of different implant insertion torque on the osseointegration between dental implants and bone.Methods:8 BLBⅢimplants were inserted into mandibular premolar area in each of 4 healthy adult beagle dogs.In accordance with the Latin square experimental design methods,insertion torque at each site(2 implants were implanted at each site)was≤15 N·cm(group A),25 N·cm(group B),35 N·cm(group C)and≥45 N·cm(group D),respectively.32 implants were randomly divided into groupⅠ(implantation immediately after tooth extraction)andⅡ(implantation 1 month after implantation of groupⅠ).2 months after implantation,4 dogs were sacrificed after tetracycline-alizarin red double color fluorescent labeling.The samples were analyzed by X-ray observation,optical microscope observation,fluorescence observation and quantitative histological analysis.Results:All implants were stably retained.The bone mineralization ration,implant-bone ingrowth fraction and osseointegration indexes of group B and C was higher than those of group A and D(P<0.01)in both groupⅠandⅡ.Conclusion:For BLBⅢimplants with insertion torque of 25 N·cm and 35 N·cm may induce better osseointegration than that of≤15 N·cm and≥45 N·cm.
