正畸治疗2种咬合功能低下牙的牙龈退缩的研究

摘　　要：目的:探讨正畸治疗中开牙和低位牙2种咬合功能低下牙的牙龈退缩发生情况及相关影响因素。方法:纳入2014-01～2017-01在云南省第二人民医院口腔正畸科完成正畸治疗的病例298例,对患者治疗前后的牙龈退缩情况及相关因素进行统计分析。结果:低位牙患者(n=147)正畸治疗前后牙龈退缩率为40. 1%和80. 3%(P <0. 05);开牙患者(n=74)正畸治疗前后牙龈退缩率为48. 6%和75. 7%(P <0. 05);正畸治疗前及治疗后低位牙与开牙患者牙龈退缩率均无差异(P> 0. 017),但均高于无低位牙及开牙的患者(P <0. 017);正畸治疗后低位牙患者牙龈退缩增高程度高于其它无低位牙及开牙的患者(P<0. 017),与开牙患者无统计学差异(P> 0. 017)。咬合功能低下(低位牙,开牙)患者正畸治疗后发生牙龈退缩的风险高于其他患者,且退缩率与是否拔牙矫治、牙龈生物型、正畸前牙龈指数相关(P <0. 05)。结论:2种咬合功能低下患者正畸治疗后更易发生牙龈退缩;且进行过拔牙矫治、薄的牙龈生物型、正畸前牙龈指数高者正畸后牙龈退缩发生率高。Objective:To investigate the incidence and relevant factors of gingival recession of the teeth with two kinds occlusal hypofunction in orthodontic treatment.Methods:298 patients(aged 12 to 42)after orthodontic treatment were included.According to their models,photos,panoramic radiographs and diagnosis records,the incidences of gingival recession before and after orthodontic treatment were compared statistically.Results:The incidence of gingival recession in infraversion teeth group(n=147),before and after treatment was 40.1%and 80.3%(P<0.05),in open bite teeth group(n=74)48.6%and 75.7%(P<0.05),respectively.There were no significant difference of the gingival recession incidence between the two kinds of occlusal hypofunction group before and after orthodontic treatment(P>0.017).After orthodontic treatment the gingival recession incidence of the occlusal hypofunction groups was heigher than that of other groups(P<0.017).The related factors for the gingival recession of occlusal hypofunction group were tooth extraction,gingival biological type and the gingival index before orthodontic treatment.Conclusion:After orthodontic treatment,gingival recession is more likely to occur in the patients with occlusal hypofunction.The incidence of gingival recession after orthodontic treatment is higher in the patients underwent tooth extraction,with thin gingival and high gingival index before orthodontic treatment.
