摘　　要：目的:探讨吸烟对慢性牙周炎患者牙周指数及龈沟液(GCF)中MCP-1、IL-8表达的影响。方法:纳入慢性牙周炎患者80例,其中吸烟与非吸烟患者各40例分别进行牙周基础治疗,于治疗前及治疗4周后进行PLI、PD、BOP、AL的检查和GCF收集,采用酶联免疫吸附法(ELISA)检测GCF中MCP-1、IL-8水平。采用SPSS 19. 0进行统计分析。结果:治疗前吸烟组PLI、BOP及GCF、MCP-1、IL-8水平高于非吸烟组(P <0. 05);牙周基础治疗4周后,2组患者的牙周临床指标PLI、BOP、PD、AL和GCF及其内容物MCP-1、IL-8水平均较治疗前明显降低(P <0. 05),但吸烟组各种指标的降低幅度明显低于非吸烟组(P <0. 05)。吸烟指数与PLI、BOP、MCP-1、IL-8、GCF呈正相关(r=0. 433、0. 454、0. 379、0. 386、0. 412,P <0. 05)。结论:吸烟可能通过增加慢性牙周炎患者牙菌斑的量,上调GCF中MCP-1、IL-8的表达,从而降低牙周基础治疗的疗效。Objective:To study the effects of smoking on periodontal indexes,MCP-1 and IL-8 expression in gingival creviculr fluid(GCF)in the patients with chornic periodontitis before and after the periodontal basic treatment.Methods:80 patients with chronic periodontitis,40 smoking(group S)and 40 non-smoking(group NS),were included.Before and 4 weeks after basic periodontal treatment,tooth plaque index(PLI),probing depth(PD),bleeding on probing(BOP)and attachment loss(AL)were examined and analyzed by using SPSS 19.0 software.MCP-1 and IL-8 level in GCF were measured by ELISA.Results:Before treatment PLI,BOP,GCF and the level of MCP-1 and IL-8 in group S were higher than those in group NS(P<0.05).After treatment,all the indexes in both groups were decreased,in group S decreased less than in group NS(P<0.05).There was a positive correlation between PLI,BOP,MCP-1,IL-8,GCF and smoking index(r=0.433,0.454,0.379,0.386 and 0.412 respectively,P<0.05).Conclusion:Smoking reduces the curative effect of periodontal basal treatment by increasing PLI and the expression of MCP-1 and IL-8 in GCF in patients with chronic periodontitis.
