摘　　要：目的:分析灼口综合征(BMS)患者定量温度觉阈值(QTTs)。方法:对55名BMS患者与26名健康志愿者进行下唇、舌尖的冷感觉阈值(CDT)、热感觉阈值(WDT)、冷疼痛阈值(CPT)、热疼痛阈值(HPT)测试及焦虑、抑郁状态评估。1月后,对20名治疗后症状好转者复测QTTs。结果:与正常人相比,BMS患者下唇CPT、舌尖CDT、CPT显著降低(迟钝),舌尖HPT显著升高(迟钝);治疗后下唇CPT显著升高,舌尖HPT显著降低(P <0. 05)。舌尖CDT与抑郁有显著相关性(P <0. 05)。结论:定量温度觉检测(QTT)在BMS小纤维神经病变的诊断、评估方面具有一定应用价值,但应排除精神因素的干扰。Objective:To test the thresholds of quantitative thermal perception thresholds(QTTs)in patients with burning mouth syndrome(BMS).Methods:55 BMS patients and 26 age-and gender-matched healthy volunteers were included for the detection of cold detection thresholds(CDT),warm detection thresholds(WDT),cold pain thresholds(CPT)and hot pain thresholds(HPT)at the central of the lower lip and the tip of the tongue and their anxiety and depression were assessed.In addition,20 patients with improved clinical symptoms after treatment were retested after one month.Results:CDTs and CPTs at the tongue and CPTs at the lower lip were significantly lower(less sensitivity)and HPTs were significantly higher(less sensitivity)at the tongue in BMS patients compared to control participants.After treatment,CPTs were significantly higher(much sensitivity)at the lower lip and HPTs were significantly lower(much sensitivity)at the tongue compared to the those before treatment(P<0.05).CDTs at the tongue had correlation with depression(P<0.05).Conclusion:Quantitative thermal perception testing(QTT)has an application value in the diagnosis and assessment of small-fiber neuropathy in BMS patients.But the effects of psychology factors should also be considered.
