摘  要:目的探究平衡训练系统对帕金森病(PD)平衡障碍和跌倒的影响。方法选取中山大学附属第八医院2017年1月至2018年4月门诊及住院诊治的246例PD患者,随机分为观察组与对照组,每组123例。对照组给予常规抗PD药物治疗,观察组在药物治疗上联合Bio Dex平衡功能训练系统治疗。比较两组帕金森病评定量表(UPDRS评分)、Berg平衡量表(BBS评分)、Holden步行功能分级(Holden评分)、计时起立-行走测试(TUGT测试)结果及姿势稳定极限性(LOS评分)、静态跌倒风险指数(SFI评分)、静态跌倒风险指数(DFI评分),随访6周记录患者跌倒次数。结果治疗后观察组UPDRS评分低于对照组,差异有统计学意义(P <0. 05);治疗后观察组BBS评分、Holden评分高于对照组,TUGT测试结果小于对照组,差异有统计学意义(P <0. 05);治疗后观察组LOS评分高于对照组,DFI评分低于对照组,差异有统计学意义(P <0. 05);随访期间观察组跌倒次数显著少于对照组,差异有统计学意义(P <0. 05)。结论与常规药物治疗相比,平衡训练系统辅助治疗或可通过提高PD患者下肢稳定性及平衡能力,起到改善平衡障碍、减少跌倒发生次数的作用。Objective To explore the effects of balance training system on balance disorder and falls of Parkinson's disease(PD).Methods 246 patients with PD undergoing outpatient and inpatient treatment in our hospital from January 2017 to April 2018 were selected,they were randomly divided into observation group and control group,123 cases in each group.The control group was given conventional anti-PD drug therapy,the observation group was treated with the BioDex balance function training system in drug therapy.The Parkinson's disease rating scale(UPDRS score),Berg balance scale(BBS score),Holden walking function grade(Holden score),timed standing-walking test(TUGT test)results and posture stability limit(LOS score)),static fall risk index(SFI score),static fall risk index(DFI score)in the two groups were compared,the number of falls were recorded during 6 weeks of follow-up.Results After treatment,the UPDRS score of the observation group was lower than that of the control group(P<0.05).The BBS score and Holden score of the observation group were higher than those of the control group after treatment.The TUGT test result was smaller than that of the control group(P<0.05);After treatment,the LOS score of the observation group was higher than that of the control group,the DFI score was lower than that of the control group(P<0.05).The number of falls in the observation group was significantly lower than that of the control group during the follow-up period(P<0.05).Conclusion The adjuvant therapy of balanced training system may improve the balance disorder and reduce the number of falls by improving the stability and balance of lower limbs in patients with PD.
