血浆miR-1183在结直肠癌中的表达及临床意义

摘　　要：目的探讨miR-1183在结直肠癌组织和血浆中的表达及临床意义。方法实时荧光定量PCR检测16对结直肠癌组织和癌旁组织样本及68例结直肠癌患者、15例上皮内瘤变患者和25例健康体检者血浆样本中miR-1183的表达水平;统计学方法分析血浆miR-1183的表达与临床病理特征的相关性。结果 miR-1183的表达水平在结直肠癌组织中高于癌旁组织(P=0.002 5),在结直肠癌患者和瘤变者血浆中均高于健康者,且在结直肠癌患者血浆中高于瘤变者(均P <0.001)。结直肠癌患者血浆miR-1183表达量与临床分期、淋巴结转移、远处转移、CEA表达显著相关(均P <0.05)。血浆miR-1183作为诊断标记物,鉴别结直肠癌的曲线下面积(AUC)为0.89(95%CI:0.823 1~0.956 9,P <0.000 1),鉴别瘤变的AUC为0.845 3(95%CI:0.713 4~0.977 3,P=0.000 3)。血浆miR-1183高表达的患者总生存期明显短于低表达者(P=0.031 0),血浆miR-1183表达水平可作为结直肠癌患者预后判断的独立影响因素(P <0.05)。结论 miR-1183高表达与结直肠癌发生发展密切相关,其血浆表达水平有望成为结直肠癌辅助诊断及预后判断指标。Objective To investigate the expression and clinical significance of miR-1183 in colorectal cancertissues and plasma.Methods The expression levels of miR-1183 in 16 pairs of tissues specimens and plasma specimens of 68 colorectal cancer patients,15 intraepithelial neoplasia(HGIEN)patients and 25 healthy volun-teers were detected by real-time fluorescent quantitative-PCR.Statistical analysis was applied to analyze the corre-lation between plasma miR-1183 expression and clinicopathological characteristics.Results The expression of miR-1183 in colorectal cancer tissues was significantly higher than that in adjacent tissues(P=0.002 5).The expression levels in the plasma specimens of HGIEN and colorectal cancer patients were higher than that of healthy volunteers;moreover,the plasma expression level in colorectal cancer patients was higher than that in HGIEN patients(both P<0.001).The plasma level of miR-1183 was significantly related with clinical stage,lymph node metastasis,distal metastasis and CEA expression(all P<0.05).When plasma miR-1183 was used as a diagnostic marker,the area under the curve(AUC)for identifying colorectal cancer was 0.89(95%CI:0.823 1~0.956 9,P<0.000 1),and the AUC for identifying intraepithelial neoplasia was 0.845 3(95%CI:0.713 4~0.977 3,P=0.000 3).The overall survival of patients with high expression of miR-1183 was significantly shorter than that of patients with low expression(P=0.0310).Expression of miR-1183 could be used as independent predictor of prognosis in colorectal cancer patients(P<0.05).Conclusion Over-expression of miR-1183 is closely related to the occurrence and development of colorectal cancer and plasma miR-1183 is a promising biomarker for the early detection and prognosis of colorectal cancer.
