腹腔镜直肠癌根治术治疗高龄患者的疗效分析

摘　　要：目的探讨腹腔镜直肠癌根治术在80岁以上高龄直肠癌患者中的疗效和安全性。方法回顾性分析78例80岁以上高龄直肠癌患者的临床资料,按照手术方式分为腹腔镜组42例与开腹组36例。比较2组手术相关指标。结果腹腔镜组术后排气时间、术后下床活动时间、术后镇痛药应用例数、术后住院时间、手术时间、术中出血量、创口大小均显著优于开腹组(P <0. 05)。2组住院费用、肿瘤直径、淋巴结清扫数目、淋巴结转移数目无显著差异(P> 0. 05)。腹腔镜组术后并发症发生率为21. 4%,显著低于开腹组的58. 3%(P <0. 05)。结论对于高龄直肠癌患者,腹腔镜直肠癌根治术具有创伤小、术后恢复快、并发症发生率低等优点。Objective To explore the efficacy and safety of laparoscopic radical resection of rectal cancer in senile patients over 80 years old.Methods Clinical materials of 78 senile patients over 80 years old with rectal cancer were retrospectively analyzed.All the patients were divided into laparoscopic group(n=42)and laparotomy group(n=36)according to types of operations.Relevant surgical indicators were compared between two groups.Results In laparoscopic group,the time of exhaust after operation,the time of getting out of bed after operation,the number of analgesics used after operation,the time of hospitalization after operation,the amount of bleeding during operation and the size of wound were significantly better than those in laparotomy group(P<0.05).There were no significant differences in hospitalization expenses,tumor diameter,number of lymph node dissection and number of lymph node metastasis between the two groups(P>0.05).The incidence rate of postoperative complications in laparoscopic group was 21.4%,which was significantly lower than 58.3%in laparotomy group(P<0.05).Conclusion For senile patients with rectal cancer,laparoscopic radical resection of rectal cancer has the advantages of less trauma,faster recovery and lower incidence rate of complications.
