肝癌组织HIF-1α、MRP表达水平及与临床病理学特征的关系

摘　　要：目的探讨肝癌组织乏氧诱导因子-1α(HIF-1α)、多药耐药蛋白(MRP)表达水平及意义。方法选取2014年2月至2017年4月我院病理科保存的原发性肝细胞癌组织标本117例,同时选取40正常肝脏组织作为对照,采用免疫组化法检测HIF-1α和MRP表达。结果肝癌组织HIF-1α、MRP阳性表达率分别为72.65%和68.38%,明显高于癌旁组织(P<0.05);肿瘤包膜不完整、有门静脉癌栓、TNM分期Ⅲ～Ⅳ期、中低分化和有淋巴结转移患者HIF-1α阳性表达率分别为93.33%、94.55%、95.00%、91.67%和94.00%,明显高于肿瘤包膜完整、无门静脉癌栓、TNM分期Ⅰ～Ⅱ期、高分化和无淋巴结转移患者(P<0.05);中低分化和有淋巴结转移患者MRP阳性表达率分别为91.67%和86.00%,明显高于高分化和无淋巴结转移患者(P<0.05);肝癌组织HIF-1α表达与MRP表达呈正相关(r_s=0.737,P<0.05)。结论肝癌组织HIF-1α、MRP表达呈高表达,与患者临床病理特征有一定关系,HIF-1α、MRP之间可能存在协同作用。Objective To investigate the primary liver cancer tissue hypoxia inducible factor-1α(HIF-1α),multidrug resistance protein(MRP)expression and significance.Methods A total of 117 cases of primary hepatocellular carcinoma specimens preserved in the Department of pathology from February 2014 to April 2017 were selected,and 40 normal liver tissue were selected as the controls,immunohistochemistry was used to detect the expressions of MRP and HIF-1α.Results The positive expressions of HIF-1αand MRP in hepatocellular carcinoma tissues were 72.65%and 68.38%,significantly higher than those in adjacent tissues(P<0.05);The HIF-1αpositive expression rates in patients with tumor incomplete capsule,portal vein tumor thrombus,TNM stageⅢ-Ⅳ,middle and low differentiation and lymph node metastasis were 93.33%,94.55%,95%,91.67%and 94%,significantly higher than the tumor complete capsule,without portal vein tumor thrombus,TNM stage I-Ⅱstage,high differentiation and without lymph node metastasis(P<0.05);The positive rates of MRP expression in patients with middle and low differentiation and lymph node metastasis were 91.67%and 86.00%,respectively,higher than those with high differentiation and non lymph node metastasis(P<0.05);The expression of HIF-1αwas positively correlated with the expression of MRP(rs=0.737,P<0.05).Conclusion The expressions of HIF-1αand MRP in primary liver cancer tissue are highly expressed,which is related to the clinicopathological features of patients,and there may be synergistic effects between them.
