摘  要:目的:观察微血管减压术联合加巴喷丁治疗原发性三叉神经痛的疗效及对血清β-内啡肽(β-EP)、5-羟色胺(5-HT)、超氧化物歧化酶(SOD)水平的影响。方法:选取2017-08-2018-07本院87例原发性三叉神经痛患者。按照随机数表法分为观察组(42例)和对照组(45例),对照组采用微血管减压术治疗,观察组采用微血管减压术联合加巴喷丁治疗。比较两组患者疗效,进行视觉模拟评分法(VAS)评分,比较血清β-EP、5-HT、SOD水平以及不良反应发生率。结果:治疗2个月后,观察组总有效率显著高于对照组[92.85%(39/42)vs 66.66%(30/45)](P<0.05);血清β-EP、5-HT、SOD水平均显著高于对照组[(224.69±17.03)ng/L vs(145.38±13.97)ng/L,(49.27±6.38)μg/L vs(31.95±3.76)μg/L,(132.28±24.60)U/ml vs(110.86±22.12)U/ml](P<0.05);治疗后1个月、治疗后2个月VAS评分显著低于对照组[(2.85±0.62)分vs(4.97±0.81)分,(1.52±0.31)分vs(3.31±0.67)分](P<0.05);不良反应总发生率显著低于对照组[9.52%(4/42)vs 26.66%(12/45)](P<0.05)。结论:微血管减压术联合加巴喷丁治疗原发性三叉神经痛的疗效显著,能够有效缓解疼痛、减少疼痛发作次数,提高血清β-EP、5-HT、SOD的水平,不良反应较少,有利于预后。Objective:To observe effect of microvascular decompression combined with gabapentin in the treatment of primary trigeminal neuralgia and its effect on serum beta-EP,5-HT and SOD.Method:87 primary trigeminal neuralgia who received therapy from August 2017 to July 2018 in our hospital were selected as research objects.According to random number table,those patients were divided into the observation group(n=42)and the control group(n=45).The control group was treated with microvascular decompression,while the observation group was treated with decompression combined with gabapentin.Then the treatment efficacy,visual analogue scale score,serum levels of beta-EP,5-HT and SOD of two groups before and after treatment were compared.Results:After treatment,the total effective rate of the observation group was significantly higher than that of the control group[92.85%(39/42)vs 66.66%(30/45)](P<0.05).The serum levels of beta-EP,5-HT and SOD were significantly higher than those in control group[(224.69±17.03)ng/L vs(145.38±13.97)ng/L,(49.27±6.38)μg/L vs(31.95±3.76)μg/L,(132.28±24.60)U/mL vs(110.86±22.12)U/mL](P<0.05).The VAS score of 1 months after treatment and 2 months after treatment was significantly lower than that of the control group[(2.85±0.62)scores vs(4.97±0.81)scores,(1.52±0.31)scores vs(3.31±0.67)scores](P<0.05).The total incidence of adverse reactions was significantly lower than that of the control group[9.52%(4/42)vs 26.66%(12/45)](P<0.05).Conclusion:Microvascular decompression combined with gabapentin in the treatment of primary trigeminal neuralgia has a significant effect,can effectively relieve pain,reduce the number of times,improve trigeminal nerve-related factors,improve the serum levels of beta-EP,5-HT,SOD,reduce adverse reactions,is conducive to prognosis.
