多模态MR与CT小肠造影在小肠肿瘤性疾病诊断中的应用

摘　　要：目的比较多模态MR与CT小肠造影诊断小肠肿瘤性疾病的应用价值。方法以60例疑有小肠肿瘤性疾病患者作为研究对象,由两位有资质的影像科医师对CT和MR图像进行阅片,从肠壁形态、病变部位(十二指肠、近段回肠、远段回肠、近段空肠、远段空肠)及累及范围进行分析,并与手术及内镜活检病理结果对照。结果 60例患者经手术及内镜活检证实有33例患者存在35处小肠肿瘤性病灶(5例腺癌、6例淋巴瘤、18例间质瘤、3例神经内分泌肿瘤、2例海绵状血管瘤及1例脂肪瘤);CT对小肠肿瘤性疾病诊断的特异度、敏感度及准确度分别为100%(27/27)、90.9%(30/33)、95.0%(57/60),MR分别为100%(27/27)、97.0%(32/33)、98.3%(59/60),两者的诊断效能相近。结论多模态MR与CT小肠造影均是小肠肿瘤性疾病的有效诊断手段,值得在临床上推广应用。Objective To compare the application value of multi-modality MR enterography and CT enterography in the diagnosis of small bowel tumor.Methods From March2016to March2017,sixty patients who were suspected with small bowel tumor in our hospital were selected as the subjects.The CT and MR images were read by two qualified imaging physicians.The morphology,lesion sites(duodenum,proximal ileum,distal ileum,proximal jejunum,distant jejunum)and the range of involvement were analyzed,and the results were compared with those of surgery and endoscopic biopsy.Results Thirty-three patients(35cases of small intestinal tumor lesions)were confirmed(5cases of aden carcinoma,6cases of lymphoma,18cases of stromal tumor,3cases of neuroendocrine tumors,2cases of cavernous hemangioma and1case of lipoma)in60patients treated by surgery and endoscopic biopsy.The specificity,sensitivity and accuracy of CT in the diagnosis of small bowel tumor were100%(27/27),90.9%(30/33)and95.0%(57/60)respectively,and MR were100%(27/27),97%(32/33)and98.3%(59/60)respectively.The diagnostic efficacy was similar for both.Conclusion Both multimodality MR enterography and CT enterography are effective diagnostic methods for small bowel tumor diseases,which are worthy for clinical application.
