施他宁联合血必净治疗重症急性胰腺炎的疗效及其对患者血清细胞因子水平的影响

摘　　要：目的探讨施他宁联合血必净治疗重症急性胰腺炎的疗效及其对患者血清白细胞介素10(IL-10)、白细胞介素18(IL-18)、肿瘤坏死因子α(TNF-α)水平的影响。方法选择2016年9月～2017年2月我院接诊的80例重症急性胰腺炎患者为研究对象,按照抽签法将患者分为实验组和对照组,每组40例。对照组在常规治疗的基础上给予施他宁治疗,实验组在对照组治疗方案基础上联合血必净治疗。比较两组治疗中的临床症状减轻及恢复时间,血检结果,尤其是血清淀粉酶的指标恢复;另外,比较两组血清IL-10、IL-18、TNF-α水平。结果治疗前,两组血清淀粉酶、血尿素氮、血糖、白细胞记数,IL-10、IL-18、TNF-α比较差异无统计学意义(P>0.05);治疗后,实验组总有效率高于对照组(P<0.05),血清淀粉酶、血尿素氮、血钙、血糖、白细胞记数治疗前后差值大于对照组(P<0.05)。治疗后,两组血清IL-10显著上升,IL-18、TNF-α显著下降,实验组IL-10、IL-18、TNF-α治疗前后差值大于对照组,差异均有统计学意义(P<0.05)。结论施他宁联合血必净治疗能提高重症急性胰腺炎的疗效,能够有效恢复血清IL-10水平、降低血清IL-18、TNF-α水平。Objective To discuss the clinical efficacy of stilamin combined with Xuebijing in the treatment of severe acute pancreatitis and its effect on serum IL-10,IL-18 and TNF-α.Methods 80 patients with severe acute pancreatitis admitted to our hospital from September 2016 to February 2017 were selected as the subjects.According to the different treatment methods,thepatientsweredivided intoexperimental groupandcontrol group.Thepatients in theexperimental groupwere treatedwith stilaminon thebasisof routine treatment.The experimental group was treated with Xuebijing treatment on thebasisof thecontrol group.The clinical symptoms to reduceand recovery time,blood test results,especially serum amylase index recovery of the two groups of patients in the treat ment were observed.The serum IL-10,IL-18 and TNF-αwere compared bet ween the two groups.Results Before treatment,the serum amylase,blood urea nitrogen,blood glucose,white blood cell count,IL-10,IL-18 and TNF-αwere not statistically significantly different after treatment.The total effective rate of the experimental group(97.50%)was higher than that of the control group(80.00%)(P<0.05).The serum amylase,blood urea nitrogen,blood calcium,blood glucose,white blood cell count leukocyte count in experimental group were higher than control group(P<0.05).The serum IL-10 was significantly increased.IL-18 and TNF-αwere significantly decreased.The IL-10,IL-18 and TNF-αin the experimental group was higher than that of the control group(P<0.05).Conclusion Stilamin combined with Xuebijing treatment can improve the efficacy of severe acute pancreatitis,restore serum IL-10 levels and reduce serum IL-18,TNF-αlevels.
