摘　　要：目的探讨超声成像对甲状腺癌与甲状腺腺瘤的鉴别诊断价值。方法回顾性分析180例甲状腺占位患者的病例资料,根据疾病类型不同将患者分为甲状腺癌组(n=80)和甲状腺腺瘤组(n=100)。比较两组患者的超声形态学特征、血流动力学特征及超声弹性应变率(SR)比值。结果超声声像图中,甲状腺癌组形态不规则、边界不清、包膜不完整、回声不均、后方回声衰减、区域淋巴结肿大的比例均明显高于甲状腺腺瘤组(P﹤0.01)。甲状腺癌组的血流分级以2级、3级为主,甲状腺腺瘤组的血流分级以0级、1级为主,两组的血流分级情况比较,差异有统计学意义(P﹤0.01)。甲状腺癌组的动脉收缩期峰值流速(PSV)、阻力指数(RI)及SR比值均明显高于甲状腺腺瘤组(P﹤0.01)。结论超声成像中,甲状腺癌与甲状腺腺瘤的超声特征存在明显的差异,有助于临床鉴别诊断。Objective To analyze the value of ultrasonography in differential diagnosis of thyroid cancer and thyroid adenoma.Method One hundred and eighty patients with thyroid mass were selected and divided into thyroid cancer group(n=80)and thyroid adenoma group(n=100)according to the type of disease.The ultrasound morphology,hemodynamics and ultrasonic elastic strain ratio(SR)were compared between the two groups.Result The proportions of irregular shape,unclear boundary,incomplete capsule,uneven echo,posterior echo attenuation and regional lymph node enlargement in thyroid cancer group were significantly higher than those in thyroid adenoma group(P<0.01).The blood flow grading of thyroid cancer group was mainly grade2and grade3,while the blood flow grading of thyroid adenoma group was mainly grade0and grade1,and the difference in blood flow grading between the two groups was statistically significant(P<0.01).The ratio of peak systolic velocity(PSV),resistance index(RI)and SR in thyroid cancer group was significantly higher than that in thyroid adenoma group(P<0.01).Conclusion There are obvious differences in ultrasonographic features between thyroid cancer and thyroid adenoma,which is helpful for differential diagnosis.
