摘　　要：目的:探究阿尔茨海默病(AD)早期诊断的稳定可靠的脑脊液外周生物标志物和神经影像学指标。方法:选取家族性AD家系及散发性AD和正常对照者,检测各组脑脊液P-tau、 Aβ1-40、 Aβ1-42、 Aβ1-40/1-Aβ42,和应用核磁共振计算内颞叶结构体积。计算联合多个指标预测AD诊断的灵敏度、特异度。结果:与正常对照组比较,其余各组患者脑脊液Aβ1-40、 Aβ1-42,海马及海马旁回体积均明显减低,而Aβ1-40/1-Aβ42, p-tau181的水平升高(P <0. 05)。联合以上6个指标预测AD的灵敏度为96%、特异度为90%。结论:联合检测以上6个指标有可能早期诊断AD。objective To investigate reliable biomarkers of cerebrospinal fluid and neuroimaging in the early diagnosis of Alzheimer's disease(AD). Methods We recruited family AD,sporadic AD and normal control people.Values of P-tau,Aβ1-40,Aβ1-42,Aβ1-40/Aβ1-42in each group of cerebrospinal fluid patients were detected,and inner temporal lobe structure volume was calculated.The sensitivity and specificity of AD diagnosis are predicted by multifactor analysis combined with multiple indexes.Results Compared with the normal control group,the cerebrospinal fluid Aβ1-40and Aβ1-42,and the volume of hippocampus and perihippocampus in the remaining groups were all reduced,while the ratio of Aβ1-40/Aβ1-42was increased by p-tau181(P<0.05).Combined with the above six indicators,the sensitivity of AD was96%and the specificity was90%.Conclusion Combined detection all the above indexes may early diagnose AD.
