摘　　要：目的分析缺氧相关因子Glut-1与HIF-1α及p-Akt在头颈部神经内分泌癌中表达影响。方法回顾性分析接受治疗的头颈部神经内分泌癌活检或手术标本66例,选取同期接受治疗60例声带息肉患者作为对照组,免疫组织化学法分析PI3K、Glut-1、p-Akt及HIF-1α在癌症和声带息肉内表达情况,单因素和多因素分析影响患者生存率因素。结果头颈部神经内分泌癌中Glut-1阳性表达率为81. 82%(54/66),声带息肉组织共阳性表达率为6. 67%(4/60);HIF-1α表达率为54. 44%(36/66),声带息肉表达率为0; PI3K阳性表达率为45. 45%(30/66),声带息肉表达率为0; pAkt表达率为36. 36%(24/66),声带息肉表达率为0,差异均有统计学意义(χ~2=10. 984、9. 387、12. 063、11. 338,P <0. 05); 66例头颈部神经内分泌癌患者3年生存率36. 36%,单因素分析显示是否出现淋巴结转移与远处转移患者间生存率对比差异有统计学意义(P <0. 05)。结论头颈部神经内分泌癌患者PI3K、Glut-1、p-Akt及HIF-1α表现为高表达,其生存情况和发生远处转移有联系。Objective To analyze the expression of Glut-1,HIF-1 alpha and p-Akt in head and neck neuroendocrine carcinoma.Methods A retrospective analysis from June 2011 to June 2014 in our hospital for treatment of head and neck neuroendocrine carcinoma biopsy or surgical specimens of 66 cases were selected,treated 60 cases of vocal cord polyp patients as the control group in our hospital,the expression of PI3K,Glut-1,p-Akt and HIF-1 in cancer and polyp of vocal cord in immunohistochemistry,single factor and multi factor analysis of the factors affecting the survival rate of patients.Results The positive expression rate of Glut-1 was 81.82%in the head and neck in neuroendocrine carcinoma(54/66),the positive expression rate of 6.67 vocal cord polyps(4/60),HIF-1 expression rate was 54.44%(36/66),vocal cord polyp expression rate was 0%,the positive expression rate of PI3K was 45.45%(30/66),vocal cord polyp expression rate was 0%,the expression rate of p-Akt was 36.36%(24/66),vocal cord polyp expression rate was 0%,the differences were statistically significant(χ2=10.984,9.387,12.063,11.338,P<0.05);66 cases of head and neck in patients with neuroendocrine carcinoma three years survival rate was 36.36%.The single factor analysis showed the presence of lymph node metastasis and distant metastasis among the patients survival rate difference was statistically significant(P<0.05).Conclusion PI3K,Glut-1,p-Akt and HIF-1 alpha are highly expressed in patients with head and neck neuroendocrine carcinoma,and their survival is related to distant metastasis.
