荧光素钠引导手术切除脑恶性胶质瘤临床应用价值分析

摘　　要：目的探讨荧光素钠引导手术切除脑恶性胶质瘤的临床应用价值。方法选取行手术治疗的脑恶性胶质瘤患者60例,按手术方式不同随机分为2组,观察组用荧光素钠引导手术,对照组行常规显微镜手术治疗。对比两组切除情况,功能区切除患者术前术后卡氏评分(KPS)结果,手术时间、住院时间,并发症及复发率。随访3～12个月。结果观察组整体情况全切率(83. 33%)较对照组(60. 00%)高,肺功能区及功能区全切率(90. 91%、62. 50%)均较对照组(9. 09%、37. 50%)高;观察组功能区切除患者术前术后KPS分差值较对照组高;观察组住院时间及手术时间均较对照组短;观察组并发症发生率(20. 00%)、复发率(16. 67%)均低于对照组(53. 00%、33. 33%)。差异均有统计学意义(P均<0. 05)。结论与常规显微镜下脑恶性胶质瘤切除术相比,增加荧光素钠引导手术具有缩短手术时间,提高全切率,促进功能区切除患者术后功能状态,缩短住院时间的优势,能减少并发症和复发率。Objective To analyze the clinical value of sodium fluorescein guided surgery for malignant glioma of the brain.Methods 60 patients with malignant gliomas who underwent surgical treatment were selected,and according to the different methods of operation,they were randomly divided into 2 groups,The observation group was treated with fluorescein sodium and the control group was treated by conventional microscope.The excision cases,the preoperative and postoperative KPS scores,the operation time,length of stay,complication and recurrence rate were compared between the two groups.The patients were followed up for 3~12 months.Results In the observation group,the overall situation of the total removal rate(83.33%)was higher than that in the control group(60.00%),pulmonary function area and function area total removal rate(90.91%,62.50%)were higher than those in the control group(9.09%,37.50%).The KPS scores,before and after resection of functional area in the observation group were higher than that in the control group.The operation time and hospitalization time in the observation group were shorter than those in the control group.The incidence of complications(20%),the recurrence rate(16.67%)in observation group were lower than those in the control group(53%,33.33%).The differences were statistically significant(P<0.05).Conclusion Compared with the conventional microscope malignant glioma resection,fluorescein sodium guided surgery had shorter operative time,improve the total removal rate,it can promote the function of functional area resection patients,shorten the hospitalization time,reduce complications and recurrence rate.
