经额下入路手术切除鞍结节脑膜瘤的临床效果及复发的影响因素分析

摘　　要：目的探讨经额下入路手术切除鞍结节脑膜瘤的临床效果及影响复发的因素。方法收集80例鞍结节脑膜瘤患者,均行经额下入路手术治疗,术后随访2～7. 5年,观察手术效果及复发情况,并采用非条件Logistic回归对年龄、性别、肿瘤直径、肿瘤形态、神经血管包裹程度、病理分级、手术切除程度、肿瘤质地、是否脑干水肿及术前KPS评分与肿瘤复发是否相关进行分析。结果所有患者术后72 h行颅脑增强MRI检查肿瘤切除情况,达到SimpsonⅠ级58例,Ⅱ级13例,Ⅲ级9例。11例复发,复发率为13. 8%。肿瘤直径、手术切除程度是影响鞍结节脑膜瘤术后复发的独立危险因素。结论经额下入路手术切除鞍结节脑膜瘤的总体疗效较好,术后复发率较低。对于肿瘤直径较大、手术切除程度低的患者应警惕复发的危险。Objective To investigate the clinical effect of resection of tuberculum sellae meningiomas via inferior frontal approach and the factors affecting recurrence.Methods Eighty cases of saddle tubercle meningioma were collected,and dll the patients were treated by trans frontal approach.They were followed up for 2~7.5 years and to observe the effect of operation and recurrence.The non conditional Logistic of age,gender,tumor size,tumor morphology,degree of coating,nerve and blood vessel pathological grade,surgical degree,tumor texture,brainstem edema and preoperative KPS score and recurrence relations of regression analysis.Results All patients underwent 72 h enhanced MRI examination after operation,58 cases were SimpsonⅠgrade,13 cases were SimpsonⅡgrade,9 cases were SimpsonⅢgrade.Recurrence were 11 cases,and the recurrence rate was 13.8%.Tumor diameter and extent of resection were independent risk factors for postoperative recurrence of tuberculum sellae meningioma.Conclusion The resection of tuberculum sellae meningiomas via the inferior frontal approach has good overall curative effect and low recurrence rate,and the risk of recurrence should be considered for patients with larger tumor size and lower resection level.
