摘　　要：目的探讨核磁共振成像(MRI)联合前列腺特异性抗原(PSA)在前列腺癌诊断和治疗效果评估中的应用。方法以164例疑似前列腺癌患者为研究对象,对经病理学检查证实的62例前列腺癌患者行内分泌治疗。对比MRI检查和PSA检查以及2种检查方法联合应用于前列腺癌诊断和治疗效果评估中的效果。结果 3种诊断方法的灵敏度、特异度和准确度差异均有统计学意义(P <0. 05),MRI联合PSA的灵敏度、特异度和准确度高于PSA和MRI(P <0. 05),MRI的灵敏度、特异度和准确度高于PSA(P <0. 05)。前列腺癌患者治疗后的PSA水平和前列腺体积均低于治疗前(P <0. 05)。3种诊断方法评估治疗有效率的差异有统计学意义(P <0. 05),MRI联合PSA的评估治疗有效率高于PSA和MRI(P <0. 05),PSA和MRI的评估治疗有效率差异无统计学意义(P> 0. 05)。结论 MRI联合PSA可提高对前列腺癌诊断和治疗效果评估的准确率,但在前列腺癌的临床诊治过程中,应综合考虑患者病情、经济情况和医疗条件等情况,充分发挥各种检查的优势。Objective To investage the application of MRI combined PSA in the diagnosis and treatment of prostate cancer.Methods A total of 164 suspected prostate cancer patientstreated were studied.62 cases were confirmed by pathological examination and treated with endocrine therapy.MRI,PSA and MRI combined PSA were used in the diagnosis and treatment efficient assessment of prostate cancer were compared.Results The difference in sensitivity,specificity and accuracy of three methods had statistical significance(P<0.05),the sensitivity,specificity and accuracy of MRI combined PSA were higher than those of MRI and PSA(P<0.05),the sensitivity,specificity and accuracy of MRI were higher than those of PSA(P<0.05);PSA level and prostate volume after treatment were higher than those before treatment(P<0.05);Three kinds of diagnostic methods for evaluating treatment efficient difference was statistically significant(P<0.05),MRI combined PSA for evaluating treatment efficient was higher than that of MRI and PSA(P<0.05),the difference in evaluating treatment efficient of MRI and PSA had no statistical significance(P>0.05).Conclusion MRI combined PSA can improve the accuracy of diagnosis and treatment efficient assessment of prostate cancer.However,illness,economic conditions and medical conditions should be taken into account comprehensively in the process of the clinical diagnosis and treatment.
