免疫组库测序在淋巴血液系统肿瘤诊断及疗效预测中的应用价值

摘　　要：免疫组库是指在任何指定的时间,个体循环系统中所有功能多样性的T细胞和B细胞的总和,其具体特征反映了机体免疫系统对抗各种抗原和预防疾病发生的能力。近年来,多重聚合酶链式反应和高通量测序技术的发展再次推动了研究者对免疫组库的深入研究。通过对免疫组库测序可以早期发现急性白血病缓解后的微小残留病灶,发现疾病特异的免疫球蛋白重链(IgH)重排;在新药的临床试验中,通过对免疫组库高通量测序可以发现预测药物疗效的免疫组库变化,如免疫组库多样性的增减、特异克隆型的出现或消失等;在造血干细胞移植后机体免疫重建过程中,临床医生也可以通过监测免疫组库而及时准确地了解患者免疫功能的恢复情况。本文就免疫组库测序在淋巴血液系统肿瘤的诊断及疗效预测中的应用情况进行综述。The immune repertoire refers to,at any given time,the sum of T cells and B cells with functional diversity in circulating system of an individual;the characteristics of the immune repertoire represent the body's immunity to fight various antigenic attacks and prevent the development of diseases.In recent years,with the development of multiple PCR and high-throughput sequencing technology,the study of immune repertoire has been promoted greatly.It has been found that the reoccurrence of minimal residual disease after acute leukemia remission and disease-specific immunoglobulin heavy chain(IgH)rearrangement could be early detected through this sequencing method.Besides,in clinical trials of new drugs,immunological changes in peripheral blood,such as the diversity or specific clones of immune repertoire were observed,which may explain the mechanisms of efficacy and prognosis of certain patients receiving such treatment.In the process of immune reconstruction after hematopoietic stem cell transplantation,the recovery degree of patients'immune function can also be timely and accurately grasped through the monitor of immune repertoire.This article aims to review the application value of immune repertoire sequencing in the diagnosis and outcome prediction of lymphatic and hematological malignancy.
