超早期动脉溶栓治疗缺血性脑卒中的效果

摘　　要：目的探讨超早期动脉溶栓治疗缺血性脑卒中的效果。方法选取2016年1月～2018年3月我院收治的60例急性缺血性脑卒中患者作为研究对象,按照随机数字表法将其分为观察组与对照组,每组各30例。对照组患者给予静脉溶栓治疗,观察组患者给予动脉溶栓治疗,比较两组患者的血管再通情况、尿激酶用量及治疗前后美国国立卫生研究院卒中量表(NIHSS)评分。结果观察组患者的血管总再通率为93.3%,高于对照组的73.3%,差异有统计学意义(P<0.05)。观察组患者的尿激酶用量为(76.6±18.4)U,少于对照组的(94.7±23.5)U,差异有统计学意义(P<0.05)。两组患者治疗前的NIHSS评分比较,差异无统计学意义(P>0.05);两组患者治疗后1、4、7周的NIHSS评分均低于治疗前,差异有统计学意义(P<0.05);观察组患者治疗后1、4、7周的NIHSS评分均低于对照组,差异有统计学意义(P<0.05)。结论采用动脉溶栓超早期治疗缺血性脑卒中患者,溶栓效果显著,患者神经功能恢复良好,具有较高的临床应用价值。Objective To investigate the effect of super-early arterial thrombolysis in the treatment of ischemic stroke.Methods A total of 60 cases of patients with acute ischemic stroke treated in our hospital from January 2016 to March 2018 were selected as objects and divided into the observation group and the control group according to the random number table method,with 30 cases in each group.The control group was treated with intravenous thrombolysis,while the observation group was treated with thrombolysis.The vascular recanalization,the amount of Urokinase,and the national institute of health stroke scale(NIHSS)scores were compared between the two groups.Results The total vascular recanalization rate of the patients in the observation group was 93.3%,which was higher than that in the control group(73.3%),and the difference was statistically significant(P<0.05).The amount of Urokinase in the observation group was(76.6±18.4)U,which was less than that in the control group accounting for(94.7±23.5)U,and the difference was statistically significant(P<0.05).There were no significant differences in the NIHSS scores between the two groups before treatment(P>0.05).The NIHSS scores at 1,4,and 7 weeks after treatment were lower than those before treatment,and the differences were statistically significant(P<0.05).The NIHSS scores at 1,4,and 7 weeks after treatment in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Super-early arterial thrombolysis in the treatment of patients with ischemic stroke is effective,and the patients′neurological function is well recovered,with high clinical value.
