机器人辅助后腹腔镜下肾部分切除术选择性阻断肾动脉与不阻断肾动脉的短期临床观察

摘　　要：目的比较机器人辅助后腹腔镜下肾部分切除术选择性阻断肾动脉与不阻断肾动脉的短期临床效果。方法选取济南军区总医院泌尿外科2014年1月至2018年1月收治的局限性肾恶性肿瘤的患者120例,根据手术方式的不同分为选择性阻断组(n=60)和不阻断组(n=60),对比2组患者平均手术时间、术中出血量、术后引流管拔除时间、术后排气时间以及住院天数等。结果研究结果显示,所选患者腹腔镜肾部分切除术均顺利完成,术后病理切缘均阴性,两组在术中出血量,术后引流管拔除时间、术后引流量无统计学差异;住院天数,术后GFR值具有统计学差异(P=0.001)结论位置表浅的外生性背侧TIa患者建议进行不阻断肾动脉机器人辅助后腹腔镜肾部分切除术,不增加手术时间,术中出血量增加,该手术方法安全。Objective to compare the short-term clinical effects of partial nephrectomy with or without renal artery occlusion by robotic assisted retroperitoneal laparoscopic partial nephrectomy.Methods choose XXX hospital uropoiesis surgical department in January 2014 to January 2018 were the limitations of 120 patients with renal malignant tumors,according to the operation method of divided into selective blocking group(n=60)and not blocking group(n=60),compared two groups patients average operation time,intraoperative blood loss,postoperative drainage tube removal time,postoperative exhaust time and hospitalization days.Results the results showed that the partial laparoscopic nephrectomy of the selected patients was successfully completed,and the postoperative pathological cutting margin was negative.There was no statistically significant difference between the two groups in intraoperative blood loss,postoperative drainage time and postoperative drainage flow.Conclusion for patients with superficial exogenous dorsal TIa,laparoscopic partial nephrectomy without blocking the renal artery assisted by the robot is recommended.The operation time is not increased,and intraoperative blood loss is increased,which is safe and efficient.
