绝经期宫颈癌患者术后近期尿动力学分析

摘　　要：目的分析绝经期宫颈癌患者术后近期下尿路功能的变化特点,探讨该类患者术后近期尿动力学检查的意义。方法对因宫颈癌Ⅰb1～Ⅱa期收住院的93例患者,进行根治术前及术后近期尿动力学检查,比较绝经期与非绝经期宫颈癌患者术后近期下尿路功能变化的差异。结果绝经组术后膀胱感觉缺失以及逼尿肌受损的发生率均明显高于非绝经组(P<0.05)。术后绝经组最大尿流率时逼尿肌压力明显低于非绝经组(P<0.05),术后初感容量、残余尿量均明显高于非绝经组(P<0.05)。结论绝经期宫颈癌患者下尿路功能的代偿能力相对弱,术后近期下尿路功能改变明显,易发生下尿路功能障碍。对绝经期宫颈癌患者进行根治术后近期尿动力学分析,有助于指导该类患者下尿路功能障碍的诊治。Objective To analyze the features of recent postoperative changes of urinary tract function in menopausal patients with cervical carcinoma,and to explore the significance of recent postoperative urodynamic examination in these patients.Methods Ninety-three patients withⅠb1~Ⅱa stage cervical carcinoma suffering radical hysterectomy in our hospital were conducted preoperative and postoperative urodynamics examination,in order to compare the differences of lower urinary tract function in the recent postoperative period between the menopausal patients and the non-menopausal patients.Results The incidences of sensory deficiency and detrusor injury were significantly higher in menopausal group than those in non-menopausal group after operation(P<0.05).The pressure at the maximum flow rate in menopausal group was significantly lower than that in non-menopausal group(P<0.05).The initial sensory capacity and residual urine volume in menopausal group were significantly higher than those in non-menopausal group(P<0.05).Conclusion The compensation ability of lower urinary tract function is relatively weak in menopausal patients with cervical carcinoma.The recent postoperative function changes of the lower urinary tract are obvious,and the lower urinary tract dysfunction is easy to occur in these patients.The recent postoperative urodynamic analysis is helpful to guide the diagnosis and treatment of lower urinary dysfunction in the menopausal patients with cervical carcinoma.
