加味花生衣干预治疗吉西他滨联合顺铂方案引起血小板降低的效果及费用调查分析

摘　　要：目的:观察加味花生衣提前干预吉西他滨联合顺铂方案引起血小板降低的疗效及费用。方法:选取我院180例行GP方案化疗第一疗程出现血小板减少症的恶性肿瘤患者作为观察对象,第二疗程开始时随机分为观察组88例,对照组92例,两组均给予GP方案(吉西他滨1 g/m^2,d_(1、8);顺铂30 mg/m^2,d_(1～3); 21天为1周期);观察组患者进行肿瘤化疗前1～2天开始给予加味花生衣;对照组无任何预防措施。观察两组患者化疗后4～14 d血小板的变化情况、血小板计数<75×10~9/L的时间、患者住院天数、干预血小板减少的费用等。结果:观察组和对照组患者在年龄、性别、病种、KPS评分等方面比较无统计学差异(P> 0. 05);观察组和对照组2周期化疗后血小板计数第4 d[(113. 48±41. 99)×10~9/L和(96. 70±32. 63)×10~9/L,P≤0. 05];第14 d[(88. 48±40. 88)×10~9/L和(70. 97±26. 72)×10~9/L,P≤0. 05];血小板<75×10~9/L的持续时间为[(4. 97±5. 36) d和(8. 07±6. 90) d,P <0. 05]。使用白介素的数量观察组比对照组少,使用白介素的费用观察组明显少于对照组,差异有统计学意义(P <0. 05)。患者住院天数为(15±5) d,两组无显著差异。结论:加味花生衣提前干预,可以降低抗肿瘤药物化疗引起的血小板降低的程度、缩短恢复时间、提高生存质量、减轻患者经济负担、节约有限的医疗保险资源,值得临床推广。Objective:To observe the curative effect and the cost of modified peanut clothing intervention in the platelets decreased,caused by gemcitabine and cisplatin chemotherapy regimen.Methods:180 patients with malignant tumor with thrombocytopenia in the previous course of GP regimen chemotherapy in our hospital were selected as observation objects.At the beginning of the second course,180 patients were randomly divided into the observation group(n=88)and the control group(n=92),both groups were given GP regimen(Gemcitabine 1 g/m 2,d 1,8.Cisplatin 30 mg/m 2,d 1~3.21 days for 1 cycle).Patients in the observation group were given extra peanut clothing 1~2 days before chemotherapy.The control group did not have any preventive measures.The changes of platelet count,the time that platelet count is less than 75×10 9/L,hospitalization days and the costs for thrombocytopenia in the two groups were observed at 4~14 days after chemotherapy.Results:The observation group and the control group have no statistical difference in age,gender,disease type and KPS score(P>0.05).The platelet count in the observation group and the control group after 2 cycles of chemotherapy at fourth days is[(113.48±41.99)×10 9/L and(96.70±32.63)×10 9/L,P≤0.05].At the fourteenth days is[(88.48±40.88)×10 9/L and(70.97±26.72)×10 9/L,P≤0.05].The duration of platelets<75×10 9/L is[(4.97±5.36)d and(8.07±6.90)d,P<0.05].The dosage of interleukin in the observation group was less than that in the control group.The cost of using interleukin was significantly less in the observation group than in the control group,it shows that the difference was statistically significant(P<0.05).The hospital stay was(15±5)d,there was no significant difference between the two groups.Conclusion:Modified peanut clothing early intervention can reduce the extent of thrombocytopenia caused by antineoplastic drugs,shorten the recovery time,improve the quality of life,reduce the economic burden of patients,saving medical insurance resources,it is worthy of promotion.
