摘　　要：目的:建立和应用云随访系统平台,实现患者离院-家庭-再次入院的闭环管理,支持跨科室、跨医疗组的协作,提高患者医疗服务的安全性和医疗质量水平。方法:云随访系统采用浏览器与服务器(B/S)结构,结合移动应用程序(APP)的模式构建统一患者管理平台,采集患者相关数据,为患者在家庭的监护、监测和护理提供最佳的服务远程管理模式。结果:形成人群健康实时数据库,创建全程患者健康档案,通过云随访系统获取相关健康数据,对健康数据进一步的挖掘、分析,并给出医生的诊断建议,有效缓解医疗资源的紧张,实现院内院外一体化患者健康教育管理。结论:云随访系统平台获取量化数据,准确统计分析相关医疗信息,可实现院后随访科学规范管理以及科研项目的跟踪,促进医护患沟通,降低医患矛盾,并对疾病的治疗、预防和康复进行有效管理。Objective:To build and use a cloud follow-up system platform so as to achieve a closed-loop management for patients from leaving hospital to being in family and then again to being in hospital,and to support cooperation among various department and various medical teams,and to further enhance the safety and the level of medical quality of medical service for patients.Methods:Cloud follow up system adopted the structure of browser and servicer(B/S),and it combined with the mobile APP model to build a united patient management system,and it collected the relevant data of patients.Finally,this system provided the best service of remote management mode for care,monitoring and nursing of patient in home.Results:This system has formed a timely database about population health,and built health archive for patients who were cared in whole process.The relative healthy data could be obtained through cloud follow-up system.And through further explored and analyzed these information,doctors could grasp useful diagnostic suggestion.It could effectively relieve the shortage of medical resources and achieve integrated health education management included inside and outside of hospital for patients.Conclusion:The cloud follow-up system platform can obtain quantized data,and accurately analyze the relative medical information as statistical method.And it can achieve scientific and standard management in follow-up of outside of hospital and it also can complete tracking science research project.Besides,in promoting the communication among doctors,nurses and patients,in reducing contradiction between doctors and patients,in treatment,prevention and rehabilitation for disease,this system can implement effective management.
