离体单根管前磨牙氢氧化钙封药后采用次氯酸钠联合EDTA冲洗的效果及其封闭性能

摘　　要：目的探讨离体单根管前磨牙氢氧化钙封药后采取次氯酸钠联合EDTA冲洗的效果及其封闭性能的研究。方法随机将我院牙科250例患者分为研究组和对照组,每组125例患者(160颗离体单根管前磨牙)。研究组采取次氯酸钠与EDTA冲洗残余氢氧化钙,对照组采取次氯酸钠冲洗残余氢氧化钙,随后对两组各80颗牙进行根管充填,比较不同冲洗方法对氢氧化钙的去除效果,并分析根管各面染料渗漏长度。结果研究组患者的冠1/3、中1/3、尖1/3及总体氢氧化钙清除评分均明显低于对照组,差异均具有统计学意义(P<0.05)。研究组染料渗漏长度为(0.34±0.11)mm,明显短于对照组的(0.59±0.22)mm,差异具有统计学意义(P<0.05)。结论氢氧化钙具有良好的抗菌、抑菌性,可通过封闭牙本质小管减少细菌,但其能够被血清中的碳酸钙中和,且稳定性较差,会出现扩散或牙根内吸收等现象。采用次氯酸钠与EDTA联合进行冲洗根管,可取得较好的封闭效果,同时配合根管充填能够达到满意的治疗效果。Objective To investigate the effect of sodium hypochlorite combined with EDTA washing on extracted single-root canal premolars after calcium hydroxide sealing and its sealing performance.Methods A total of250patients in stomatology department of our hospital were randomly divided into study group and control group,with125cases(160extracted single-root canal premolars)in each group.In the study group,the residual calcium hydroxide was washed with sodium hypochlorite and EDTA,and the residual calcium hydroxide was washed with sodium hypochlorite in the control group.Subsequently,80teeth in each group were filled with root canal.The effects of different washing methods on the removal of calcium hydroxide were compared,and the dye leakage length of root canals was analyzed.Results The scores of crown1/3,middle1/3,tip1/3and total removal of calcium hydroxide in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The dye leakage length in the study group was(0.34±0.11)mm,which was significantly shorter than(0.59±0.22)mm in the control group,and the difference was statistically significant(P<0.05).Conclusion Calcium hydroxide has good antimicrobial and bacteriostatic properties,which can reduce bacteria by closing dentinal tubules,but it can be neutralized by calcium carbonate in serum and its stability is poor.It can be diffused or absorbed in root.Sodium hypochlorite combined with EDTA for root canal washing can achieve good sealing effect,and root canal filling can achieve satisfactory therapeutic effect.
