布地奈德直肠用药治疗远端溃疡性结肠炎的疗效

摘　　要：目的探讨布地奈德直肠用药治疗远端溃疡性结肠炎的临床疗效。方法将86例远端溃疡性结肠炎患者按治疗方法的不同分为研究组和对照组,每组43例。2组均给予纠正水、电解质紊乱和口服免疫制剂,同时给予流质或半流质饮食。在此基础上,研究组给予布地奈德混合液保留灌肠治疗,对照组给予地塞米松混合液保留灌肠治疗。观察2组治疗4周后血清细胞间黏附分子-1(ICMA-1)、P-选择素(P-sel)水平及不良反应(胃肠道不适、肝功能异常)发生率、临床疗效。结果研究组总有效率为93.0%,明显高于对照组的76.7%(P<0.05)。研究组不良反应发生率为2.3%,明显低于对照组的14.0%(P<0.05)。研究组治疗4周后血清ICAM-1、P-sel水平均明显低于对照组(P<0.05)。结论布地奈德直肠用药治疗远端溃疡性结肠炎具有较好的疗效。Objective To explore the clinical effect of rectal administration of budesonide on distal ulcerative colitis.Methods A total of86patients with distal ulcerative colitis were divided into two groups,with43patients in each group.Both groups were given the correction of water and electrolyte disorders,oral immunological preparations,and liquid or semi-liquid diet.In addition,the study group and control group were treated with retention enemas budesonide mixture and dexamethasone,respectively.The levels of serum intercellular adhesion molecule-1(ICMA-1)and P-selectin(P-sel),incidence of adverse reactions(gastrointestinal discomfort and liver dysfunction)and curative effect were observed after4weeks of treatment.Results The total effective rate in study group(93.0%)was higher than that in control group(76.7%)(P<0.05).The incidence of adverse reactions in study group(2.3%)was lower than that in control group(14.0%)(P<0.05).After treatment for4weeks,serum ICAM-1and P-sel levels in study group were lower than those in control group(P<0.05).Conclusion Rectal administration of budesonide is effective for distal ulcerative colitis.
