无托槽隐形矫治与传统固定矫治在错颌畸形患儿正畸治疗中的对比观察

摘　　要：目的观察无托槽隐形矫治在错颌畸形患儿正畸治疗中的效果。方法选取需行正畸治疗的错颌畸形患儿82例作为研究对象,根据治疗方法的不同分为观察组39例和对照组43例,观察组给予无托槽隐形矫治,对照组给予传统固定矫治,均治疗6个月。比较2组临床疗效,治疗前后2组牙周指数[牙龈指数(gingival index,GI)、龈沟探诊深度(sulcus probing depth,SPD)、龈沟出血指数(sulcus bleeding index,SBI)、菌斑指数(plaque index,PLI)],2组倾斜牙扶正时间、转位牙扭正时间及治疗完成所需时间,矫治后满意度,治疗期间不良反应发生情况。结果观察组临床疗效优于对照组,总有效率高于对照组(P<0.05)。治疗后,对照组PLI、SBI明显高于治疗前,观察组PLI、SBI明显低于对照组(P<0.05)。观察组倾斜牙扶正时间、转位牙扭正时间明显短于对照组(P<0.05)。观察组美观舒适、固位功能、咀嚼功能、便捷程度及语言功能满意度评分明显高于对照组(P<0.05)。观察组治疗期间口腔黏膜损伤、龋病及牙龈炎发生率明显低于对照组(P<0.05)。结论无托槽隐形矫治在错颌畸形患儿正畸治疗中具有较好的效果,有利于保持口腔卫生,不影响牙周健康,并可提高患者满意度,减少治疗期间口腔不良反应的发生,值得推广应用。Objective To explore the effect of invisible brackets in orthodontic treatment of children with malocclusion.Methods Eighty-two children with malocclusion who needed orthodontic treatment were divided into observation group(39cases)and control group(43cases)according to different treatment methods.The observation group was treated with invisible correction without bracket,and the control group was treated with traditional fixation for6months.The clinical effects of the two groups were compared.The periodontal index(GI),sulcus probing depth(SPD),sulcus bleeding index(SBI)and plaque index(PLI)of the two groups were compared before and after treatment.The orthodontic time of inclined teeth,orthodontic time of transposition teeth and the end of treatment were compared.The time needed for completion,the satisfaction after correction,and the incidence of adverse reactions during treatmentwere also compared.Results The clinical efficacy of the observation group was better than that of the control group,and the total effective rate was higher than that of the control group(P<0.05).After treatment,the levels of PLI and SBI in the control group were significantly higher than those before treatment,while those in the observation group were significantly lower than those in the control group(P<0.05).The righting time of the tilted teeth and the time of twisting teeth were significantly shorter in the observation group than in the control group(P<0.05).The satisfaction scores of aesthetic comfort,retention function,masticatory function and language function in the observation group were significantly higher than those in the control group(P<0.05).The incidence of oral mucosal injury,caries and gingivitis in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Stealth orthodontic treatment without bracket has a good effect in the orthodontic treatment of children with malocclusion.It is helpful to maintain oral hygiene,does not affect periodontal health,and can improve patient satisfa
