摘　　要：目的探讨腹腔镜与开腹手术治疗早期胃癌临床效果对比。方法将2014年1月至2015年1月98例早期胃癌患者根据数字表法分对照组和腹腔镜组,每组49例。对照组采取开腹手术治疗,腹腔镜组采取腹腔镜手术治疗。比较两组术后异常现象发生率;切口长度、排气恢复时间、术后禁食时间、住院时间;淋巴结清扫总数、手术操作时间、2年复发率。结果腹腔镜组术后异常现象发生率低于对照组(P<0.05);腹腔镜组切口长度、排气恢复时间、术后禁食时间、住院时间短于对照组(P<0.05);两组淋巴结清扫总数、手术操作时间、2年复发率差异无统计学意义。结论腹腔镜与开腹手术治疗早期胃癌均有良好效果,远期复发率低,淋巴结清扫彻底,但腹腔镜手术胜在创伤更轻,并发症更少,术后康复更快。Objective To compare the clinical effects of laparoscopic and open surgery in the treatment of early gastric cancer.Methods From January2014to January2015,98patients with early gastric cancer were divided into control group and laparoscopic group according to the digital table method,with49cases in each group.The control group was treated with open surgery and the laparoscopic group was treated with laparoscopic surgery.The incidence of postoperative abnormalities was compared between the two groups;incision length,exhaust recovery time,postoperative fasting time,hospital stay;total lymph node dissection,surgical operation time,and2year recurrence rate.Results The incidence of abnormalities in the laparoscopic group was lower than that in the control group(P<0.05).The incision length,exhaust recovery time,postoperative fasting time,and hospitalization time in the laparoscopic group were shorter than those in the control group(P<0.05);There was no significant difference in the total number of lymph node dissection,operation time and2year recurrence rate between the two groups.Conclusion Laparoscopic and open surgery have good results in the treatment of early gastric cancer.The long-term recurrence rate is low and the lymph node dissection is thorough.However,laparoscopic surgery is better than trauma,less complications,and faster postoperative recovery.
