摘　　要：目的通过观察对比患者的不同临床表现,探讨LEEP刀宫颈电切术在保留宫颈癌患者生育能力及改善妊娠结局中的效果。方法选取2015年4月～2016年4月在我院接受治疗的140例宫颈癌患者,按照随机数字表法将其分为观察组和对照组,两组均为70例,其中对照组患者行宫颈锥切术,观察组患者行LEEP刀宫颈电切术,观察记录两组患者的相关临床指标数据。结果对比两组患者的妊娠率,观察组明显较对照组要高,差异有统计学意义(P <0.05);对比两组患者的足月分娩率、早产率以及流产率,差异无统计学意义(P> 0.05);对比两组患者的顺产率,观察组较对照组要高,剖宫产率则低于对照组,差异有统计学意义(P <0.05);对比两组患者的臀助产率、吸引产率以及产钳助产率,差异无统计学意义(P> 0.05);对比两组患者的产程和出血量,观察组均少于对照组,差异有统计学意义(P <0.05);对比两组患者的新生儿出生体重,差异无统计学意义(P> 0.05)。结论与传统的宫颈锥切术相比,LEEP刀宫颈电切术在保留宫颈癌患者生育能力及改善妊娠结局中的效果显著,可有效提升患者的顺产率和降低剖宫产率,且具有一定的安全性,因而可在临床上进行推广应用。Objective To explore effect of LEEP knife cervical resection on maintaining fertility and improving pregnancy outcome in patients with cervical cancer by observation and comparison of different clinical manifestations of patients.Methods One hundred and forty patients with cervical cancer who were treated in our hospital from April 2015 to April 2016 were selected and they were divided into the observation group and the control group according to random number table method,with 70 cases in each group.Patients in the control group were given conization of cervix while patients in the observation group were given LEEP knife cervical resection.Data of relevant clinical indicators of patients in two groups were observed and recorded.Results Pregnancy rate of the observation group was significantly higher than that of the control group and the difference was statistically significant(P<0.05).Term delivery rate,premature birth rate and abortion rate of patients in two groups were compared and the difference had no statistical significance(P>0.05).Spontaneous labor rate of the observation group was higher than that of the control group while cesarean section rate was lower than that of the control group.The difference was statistically significant(P<0.05).Differences in hip delivery rate,extractor delivery rate and forceps delivery rate in two groups had no statistical significance(P>0.05).The time of labor and blood loss of the observation group were both less than those of the control group and the difference was statistically significant(P<0.05).Difference in neonatal birth weight of two groups had no statistical significance(P>0.05).Conclusion Compared with traditional conization of cervix,LEEP knife cervical resection has a significant effect on maintaining fertility and improving pregnancy outcome.It can effectively improve spontaneous labor rate and reduce cesarean section rate and it has certain security.Therefore,it can be clinical application and promotion.
