经尿道膀胱瘤电切术加药灌注治疗浅表性膀胱瘤的临床疗效观察

摘　　要：目的探讨对浅表性膀胱癌患者采取经尿道膀胱瘤电切术加药灌注治疗的临床疗效。方法本次研究选取我院在2014～2016年期间收治的80例浅表性膀胱癌患者,按照随机数字表法将其分为治疗组与对照组,每组患者各40例。治疗组患者采取经尿道膀胱瘤电切术加药灌注治疗,对照组患者仅采取经尿道膀胱瘤电切术治疗,对其临床治疗效果与血清水平等相关指标进行比较分析。结果治疗组患者手术时间为(33.46±9.86)min,与对照组手术时间(34.08±9.76)min,差异无统计学意义(P> 0.05);治疗组患者术后留置导尿管时间为(3.29±0.49)min,明显低于对照组的(5.01±0.68)min,差异有统计学意义(P <0.05)。治疗组与对照组患者术后半年复发率分别为2.5%、5.0%(P> 0.05),而治疗组患者术后1年复发率为5.0%明显低于对照组的17.5%(P <0.05)。治疗组与对照组患者治疗1周后,治疗组患者血清中FGF、MMP-9、MMP-2以及VEGF水平明显低于对照组(P <0.05)。治疗组患者术后1个月内并发症发生率为12.5%,对照组患者并发生发生率为7.5%,差异无统计学意义(P> 0.05)。结论对浅表性膀胱癌患者实施经尿道膀胱肿瘤电切术联合灌注治疗,能够显著地缩短患者术后留置导尿管时间,同时还能在一定程度上降低患者血清因子水平与术后复发率,且无明显不良反应,治疗方式安全有效。Objective To explore the clinical curative effect oftransurethral resection of bladder tumor combined with drug infusion in treatment of superficial bladder tumor.Methods 80 patients with superficial bladder tumor who were admitted and treated in our hospital from 2014 to 2016 were selected and they were divided into the treatment group and the control group according to the random number table method,with 40 patients in each group.Patients in the treatment group were treated by transurethral resection of bladder tumor combined with drug infusion while patients in the control group were only given transurethral resection of bladder tumor for treatment.Results The operation time of patients in the treatment group and the control group was respectively(33.46±9.86)min and(34.08±9.76)min.And the difference between two groups had no statistical significance(P>0.05).Postoperative indwelling catheter time of patients in the treatment group was(3.29+0.49)min,significantly lower than that in the control group(5.01±0.68)min,and the difference was statistically significant(P<0.05).The postoperative recurrence ratesafter half year in the treatment group and the control group was 2.5%and 5.0%respectively(P>0.05).The postoperative recurrence ratesafter one year in the treatment group was 5.0%,significantly lower than that in the control group 17.5%(P<0.05).After treatment for one week,levels of FGF,MMP-9,MMP-2 and VEGF in the serum of patients in the treatment group were significantly lower than those in the control group(P<0.05).The incidence rates of complications within one month after operation in the treatment group and the control groupwere respectively 12.5%and 7.5%.The difference was not statistically significant(P>0.05).Conclusion Transurethral resection of bladder tumor combined with drug infusion in treatment of superficial bladder tumor can not only significantly shorten postoperative indwelling catheter time,but also reduce serum factor level and the postoperative recurrence rate to a certain extent.It has no s
