肝癌TACE治疗术前常规行CT腹部血管成像对插管操作的指导价值

摘　　要：目的:探讨多层螺旋CT血管重建技术在肝癌介入治疗中对插管操作的指导意义。方法:66例中晚期原发性肝癌患者,35例行腹部MSCT平扫及三期增强扫描并行CTA重建(CTA组),31例行腹部MSCT平扫及三期增强扫描,不进行CTA重建(无CTA组),并分别行TACE介入治疗,记录两组患者TACE术中导管使用数量、曝光时间、插管使用时间、造影剂用量。结果:CTA组导管使用数量和造影剂用量较无CTA组少,曝光时间和插管使用时间均明显缩短,差异具有统计学意义(P <0. 05)。结论:多层螺旋CT血管重建能准确显示腹腔动脉变异、肿瘤供血动脉和病变情况,对TACE插管操作有重要的指导意义。Objective:To explore the guiding value of MSCT vascular reconstruction technique for intubation in the TACE intervention therapy of liver cancer.Methods:66cases of patients with advanced primary liver cancer were selected,in which35cases were given abdominal MSCT scan and three phase enhanced scan combined with CTA reconstruction(CTA group),and31patients underwent abdominal MSCT scan and three phase enhanced scanning,without CTA reconstruction(no CTA group).TACE interventional treatment underwent in each group.Quantity of catheter,time of exposure,time of intubation,dosage of contrast agent between the two groups were compared.Results:The quantity of catheter and dosage of contrast agent in CTA group were less than those in no CTA group,the exposure time and intubation time were significantly shorter,and the difference was statistically significant(P<0.05).Conclusion:MSCT vascular reconstruction can accurately display the variation of the celiac artery,the blood supply artery and the pathological changes of the tumor,which has important guiding significance for TACE intubation.
