摘 要:目的探讨吲哚菁绿导航技术联合亚甲蓝在乳腺癌前哨淋巴结活检(SLNB)中的临床应用价值。方法选取80例乳腺癌患者作为研究对象,依据随机数字表法将患者分为观察组和对照组,每组40例。观察组患者采用吲哚菁绿导航技术联合亚甲蓝检测乳腺癌前哨淋巴结(SLN),对照组患者单用亚甲蓝检测乳腺癌SLN。比较两种检测方法的乳腺癌SLN检出率及诊断效能。结果观察组的灵敏度为100%(22/22),特异度为94.44%(17/18),约登指数为94.44%,准确率为97.50%(39/40)。对照组的灵敏度为95.24%(20/21),特异度为89.47%(17/19),约登指数为84.71%,准确率为92.50%(37/40)。观察组的灵敏度、特异度、约登指数、准确率均稍高于对照组,但差异均无统计学意义(P﹥0.05)。观察组共检出158枚SLN,平均SLN为(3.9±1.2)枚;对照组共检出94枚SLN,平均SLN为(2.3±1.1)枚;观察组检出的SLN数目多于对照组,差异有统计学意义(P﹤0.05)。结论吲哚菁绿导航技术联合亚甲蓝检测SLNB的灵敏度、特异度、约登指数及准确率均较高,能够提高SLN的检出数量,临床效果较优,应用价值较高。Objective To evaluate the clinical value of indocyanine green navigation combined with methylene blue in sentinel lymph node biopsy(SLNB)of breast cancer.Method Eighty patients with breast cancer were selected and divided into observation group and control group according to the method of random number table,40 cases in each group.The patients in observation group underwent indocyanine green navigation technology combined with methylene blue for breast cancer sentinel lymph node(SLN)detection,while the patients in control group underwent methylene blue alone for SLN detection.The detection rate and diagnostic efficacy of the two methods were compared.Result The sensitivity,specificity,Yoden index and accuracy rate of the observation group were 100%(22/22),94.44%(17/18),94.44%and 97.50%(39/40),respectively.The sensitivity,specificity,Yoden index and accuracy rate of the control group were 95.24%(20/21),89.47%(17/19),84.71%and 92.50%(37/40),respectively.The sensitivity,specificity,Yoden index and accuracy rate of the observation group were slightly higher than those of the control group,without significant difference(P>0.05).There were 158 SLN detected in the observation group,with an average of(3.9±1.2)SLN;while there were 94 SLN detected in the control group,with an average of(2.3±1.1)SLN;the number of SLN detected in the observation group was significantly more than that in the control group,with a significant difference(P<0.05).Conclusion Indocyanine green navigation combined with methylene blue in detecting SLNB with a slightly higher sensitivity,specificity,Yoden index and accuracy rate,which can improve the detected quantity of SLN,with a batter clinical effect and a higher application value.
