非肌层浸润性膀胱癌复发与吡柔比星灌注不良反应的关系

摘　　要：目的探索经尿道膀胱肿瘤切除术(TURBT)后行膀胱吡柔比星灌注化疗的非肌层浸润性膀胱癌患者肿瘤复发与灌注不良反应的关系。方法纳入行吡柔比星灌注化疗的新发膀胱非肌层浸润性尿路上皮癌80例患者,回顾性分析TURBT后1年的临床资料、病理资料、膀胱灌注化疗不良反应种类及严重程度。结果泌尿系统局部的不良反应较轻,仅6例患者(7.5%)发生泌尿系统的局部Ⅲ级药物不良反应,需要延迟膀胱灌注化疗。相对于肿瘤复发组患者而言,肿瘤未复发组患者局部不良反应(尿路刺激症状)严重程度较高[尿频(P=0.036)、尿急(P=0.034)、尿痛(P=0.042)]。结论 TURBT术后1年内,非肌层浸润性膀胱癌患者在膀胱吡柔比星灌注化疗期间,膀胱肿瘤未复发组较复发组有更严重的尿路刺激症状。Objective To explore the association between recurrence and side effects of intravesical pirarubicin chemotherapy in non-muscle invasive bladder cancer(NMIBC)patients after transurethral resection of bladder tumor(TURBT).Methods The clinical and pathological data,categories and severity of side effects of intravesical chemotherapy in 80 new NMIBC patients 1 year after TURBT were retrospectively analyzed.Results The local side effects on urinary tract were mild.Only 6 patients(7.5%)hadⅢdegree side effects and had to postpone intravesical chemotherapy.Patients in the non-recurrence group had more severe urinary irritating symptoms,including frequent micturition(P=0.036),urgent urination(P=0.034),and urodynia(P=0.042)than patients in the recurrence group(P<0.05).Conclusions NMIBC patients in the non-recurrence group 1 year after TURBT had more severe side effects such as frequent micturition,urgent urination,and urodynia than patients in the recurrence group during intravesical pirarubicin chemotherapy.
