摘　　要：目的探讨应用紫杉醇治疗乳腺癌术后患者外周血中凋亡抑制基因Survivin mRNA及血清CA15-3、HER-2表达水平的变化及辅助化疗的意义。方法收集2016年1月至2016年12月进本院接受乳腺癌切除术的患者60例及健康体检者30例,将乳腺癌患者术后随机分为化疗组(30例)和非化疗组(30例)。比较健康体检者及乳腺癌患者术前外周血中CA15-3、HER-2的表达水平,化疗组和非化疗组外周血中凋亡抑制基因Survivin mRNA、CA15-3、HER-2在术前1 d和术后120 d的表达水平。结果健康者及乳癌患者外周血中CA15-3、HER-2表达水平差异有统计学意义(P<0.05)。术前,化疗组及非化疗组外周血中凋亡抑制基因Survivin、CA15-3、HER-2表达水平差异无统计学意义(P>0. 05),术后,凋亡抑制基因Survivin表达水平与术前相比显著降低(P<0. 05),化疗组降低幅度更明显(P<0.05),CA15-3表达水平较术前显著降低(P<0.05),HER-2表达水平较术前下降,且化疗组下降更显著(P<0.05)。结论乳腺癌辅助化疗术后降低凋亡抑制基因Survivin、HER-2表达水平,提示应用紫杉醇辅助治疗乳腺癌术后具有一定的效果。外周血凋亡抑制基因Survivin及血清CA15-3、HER-2表达水平在一定程度上反映了患者的预后情况。Objective To investigate the effect of paclitaxel on the expression of survivin mRNA,serum CA15-3 and HER-2 after operation in breast cancer patients.Methods We collected 60 patients undergoing breast cancer resection from in our hospital and 30 healthy volunteers January 2016 to December 2016.Breast cancer patients were randomly divided into chemotherapy group(30 cases)and non-chemotherapy group(30 cases).The expression of CA15-3 and HER-2 in preoperative peripheral blood were compared between healthy subjects and breast cancer patients.Additionally,the Survivin mRNA,CA15-3,and HER-2 were also compared between chemotherapy and non-chemotherapy groups at 1 day before surgery and 120 days after surgery.Results There was a significant difference in the expression of CA15-3 and HER-2 in peripheral blood between healthy and breast cancer patients(P<0.05).Before surgery,the expression levels of Survivin,CA15-3 and HER-2 in the peripheral blood of the chemotherapy and non-chemotherapy groups were not statistically different(P>0.05).After the end of chemotherapy,the expression of Survivin was significantly lower than before surgery(P<0.05).The chemotherapy group decreased more significantly(P<0.05),CA15-3 expression levels decreased significantly compared with preoperative(P<0.05),HER-2 expression levels decreased compared with before surgery,and chemotherapy group decreased more significantly(P<0.05).Conclusion The reduction of Survivin and HER-2 expression after adjuvant chemotherapy for breast cancer suggests that paclitaxel has a certain effect on adjuvant breast cancer surgery.The expression of survivin and serum CA15-3 and HER-2 in peripheral blood reflect the prognosis of patients to some extent.
