摘　　要：目的检测贲门胃底癌中CIP2A mRNA表达与HER-2基因的扩增并探讨其相关性和临床意义。方法采用半定量RT-PCR法检测47例贲门胃底癌石蜡包埋组织中CIP2A mRNA的转录水平,荧光原位杂交方法(FISH)检测HER-2基因的扩增情况,统计分析CIP2A mRNA与HER-2扩增情况的相关性,同时了解二者与贲门胃底癌患者临床病理参数的关系。另选取47例对应的癌旁组织纳入半定量RT-PCR方法检测,并统计分析CIP2A mRNA在癌和癌旁组织中转录水平的差异。结果 CIP2A mRNA高水平转录与肠型胃癌、较高的分化程度、较大的肿瘤浸润深度、较高的TNM分期、血管癌栓、神经侵犯等临床病理参数相关(P <0.05);HER-2扩增与肠型胃癌、较高的分化程度、神经侵犯等临床病理参数相关(P <0.05)。CIP2AmRNA高水平转录与HER-2扩增呈正相关。贲门胃底癌组织中CIP2A m RNA的转录水平显著高于其对应的癌旁组织(P <0.05)。结论 CIP2A基因在贲门胃底癌组织中高表达,与患者的恶性临床病理参数密切相关。CIP2A基因可能成为贲门胃底癌患者新的分子标志物和治疗靶点,其可受到HER-2正调节。Objective To investigate the correlation between CIP2 A mRNA expression and HER-2 gene amplification in cardia-fundal carcinoma and to reveal the clinical significance.Methods CIP2 A mRNA level in 47 cases of cardia-fundal carcinoma was detected by RT-PCR assay. The amplification of HER-2 gene was detected by fluorescence in situ hybridzation(FISH)assay,and the correlation between CIP2 A mRNA expression and HER-2 amplification was analyzed. Meanwhile,the correlation between CIP2 A mRNA expression and HER-2 gene amplifi-cation and the clinicopathological parameters were investigated.Results The high level of CIP2 A mRNA was signif-icantly related to the clinicopathological features,such as intestinal gastric cancer,higher degree of differentiation,greater depth of tumor invasion,higher TNM stage,tumor thrombi and perineural invasion(P < 0.05,respectively),and the positive HER-2 amplification was significantly associated with intestinal gastric cancer,perineural invation and high differentiation(P < 0.05,respectively). The high expression of CIP2 A mRNA was correlated with HER-2 amplification.Conclusions CIP2 A gene is highly expressed in cardia-fundal carcinoma tissue,and is closely related to the malignant clinicopathological features of cardia-fundal carcinoma. CIP2 A gene may be a new molecular marker and therapeutic target of cardia-fundal cancer,which is positively regulated by HER-2.
