射频热凝术联合阿霉素注射治疗原发性三叉神经痛的临床分析

摘　　要：目的探究射频热凝术联合阿霉素注射治疗原发性三叉神经痛的临床疗效。方法选取2015年4月至2016年7月来我院就诊并且诊断为原发性三叉神经痛的患者105例,按照随机原则将患者分为单纯射频热凝组35例,单纯阿霉素组35例,联合治疗组35例;联合治疗组采取三叉神经射频热凝+阿霉素注射方法联合治疗。结果依次选取术后5个时间点进行随访,治疗结束后第2周和第1个月,3组差异无统计学意义(P>0.05);术后6个月、1年和2年随访时,联合治疗组的治疗总有效率高于其他2组,有效率的差异有统计学意义(P<0.05);治疗2年后,单纯阿霉素组复发2例,单纯射频热凝组复发3例,联合治疗组无复发病例。结论射频热凝术联合阿霉素注射治疗可以有效治疗原发性三叉神经痛,远期治疗有效率高于单纯使用射频热凝术和单纯阿霉素组,值得临床推广。Objective To evaluate the therapeutic effect of radiofrequency ablation and injection of epirubicin on treating primary trigeminal neuralgia(PTN).Methods A total of 105 cases of PTN selected and divided into 3 groups randomly.Groups: radiofrequency ablation and saline solution injection,injection of epirubicin to the nerve trunk only(control groups),radiofrequency ablation and injection of epirubicin to the nerve trunk(treatment group).The pain score of visual analogue scale(VAS)and clinical efficacy were compared and statistically analyzed among the 3 groups before treatment,2 weeks,1 month,6 months,1 year and 2 years after the operation.Results No statistical significances were shown 2 weeks and 1 month after operation among 3 groups(P>0.05).Compared with the control among,the total effective rate in treatment group was significantly raised in all the patients at 6 months,1 year,2 years after operation(P<0.05),there was significant difference.Post-operation recurrence of treatment group was 0,while there were 5 cases in the two control groups.Conclusion Radiofrequency ablation combined with injection of epirubicin on treating PTN is proved with lower recurrence and few complications.
