摘 要:目的探讨低剂量CT扫描技术在早期肺癌筛查中的应用与临床价值。方法于2016年1月—2018年8月对该院收治的临床可疑早期肺癌的患者100例进行观察和分析,全部患者接受常规剂量与低剂量肺部CT平扫,由两名高年资放射医师在不知病理结果的前提下分别独立阅片,对比不同剂量CT扫描技术的诊断价值。结果常规剂量CT与低剂量CT对肺癌的检出率分别为97.00%、96.00%,图像质量评分分别为(3.7±1.1)分、(3.6±0.9)分,两者比较差异无统计学意义(χ~2=0.013,P=0.906;t=1.861,P=0.062)。但是低剂量CT的权重CT剂量(CTDIw)为(1.9±0.5)mGy、有效辐射剂量(0.8±0.2)mSv均低于常规剂量CT,差异有统计学意义(t=82.072、142.232,P<0.05)。结论不论是常规剂量CT还是低剂量CT检查,都具有相当的诊断价值,但是低剂量肺部CT扫描可避免患者接受过多的辐射剂量,值得临床借鉴普及。Objective To investigate the application and clinical value of low-dose CT scanning in early lung cancer screening. Methods From January 2016 to August 2018, 100 patients with clinically suspected early stage lung cancer admitted to the hospital were ob-served and analyzed. All patients received conventional and low-dose lung CT scans. The radiologists independently read the films without knowing the pathological results, and compared the diagnostic value of different dose CT scanning techniques. Results The de-tection rates of conventional lung cancer and low-dose CT were 97.00% and 96.00%, respectively. The image quality scores were(3.7±1.1)points and(3.6±0.9)points, respectively. There was no significant difference between the two(χ~2=0.013, P=0.906; t=1.861, P=0.062). However, the weight CT dose(CTDIw) of low-dose CT was(1.9±0.5) mGy, and the effective radiation dose(0.8±0.2)mSv was lower than that of conventional dose CT. The statistical significance was established(t=82.072, 142.232, P<0.05). Conclusion Both conventional dose CT and low-dose CT examination have considerable diagnostic value, but low-dose lung CT scan can avoid pa-tients receiving excessive radiation dose, which is worthy of clinical popularization.
