尿路造口患者社会心理适应水平及自我护理能力与生活质量的相关性研究

摘　　要：目的:调查尿路造口患者的社会心理适应、自我护理能力以及生活质量水平,分析三者的相关性。方法:采用一般情况调查表、中文版造口患者社会心理适应水平量表(OAI-20)、泌尿造口患者自我护理能力测定量表、中文版造口患者生活质量量表(C-COH),对112例行根治性全膀胱切除术后尿路造口患者发放问卷进行调查,分析其相关因素。结果:本研究患者社会适应水平总均分为(53. 18±7. 88)分,处于中等水平;自我护理能力测定总均分为(12. 89±4. 02)分,生活质量总均分为(196. 89±38. 13)分,自我护理能力和生活质量水平均较低,三者之间密切相关。社会心理适应与自我护理能力正向影响生活质量(β=0. 51,0. 50; P <0. 05),同时自我护理能力作用于社会生活适应(β=0. 58,P <0. 01)。结论:在对尿路造口患者进行护理时,要注重提高患者的自我护理能力,重视其回归社会过程中的心理适应问题,从而提升患者生活质量。Objective: To investigate the psychosocial adaptation,self-care ability and the quality of life of patients with urostomy,and analyze the correlation among them. Methods: 112 patients after radical cystectomy with urostomy were investigated using general situation questionnaire,the Chinese version of Ostomy Adjustment Inventory-20( OAI-20),the scale of self-care ability for patients with urostomy,and the Chinese version of City of Hope qulity of life ostomy questionnaire( C-COH). The related factors were analyzed. Results: The average score of the social adjustment level of the patients in this study was( 53. 18 ± 7. 88),which was at a medium level. The average score of self-care ability was( 12. 89 ± 4. 02) and that of the quality of life was( 196. 89 ± 38. 13). The levels of self-care ability and the quality of life were low. There was a close correlation between the self-care ability and the quality of life. Psychosocial adaptation and self-care ability positively affected the quality of life( β = 0. 51,0. 50; P < 0. 05). At the same time,the self-care ability affects social life adaptation( β =0. 58,P < 0. 01). Conclusion: It is needed to pay attention to impoving the self-care ability of patients and the psychological adaptation in the process of integration to the society while caring the patients with urostomy,so as to improve the quality of life of patients.
