摘  要:目的:探讨时空同构结合心理治疗模式对脑梗死患者认知康复的疗效。方法:选取2016年1月-2017年1月本科室首次诊断为脑梗死且存在认知功能障碍的120例患者作为研究对象,按入院顺序将患者分为观察A组、观察B组和对照组,各40例。从入院第3天开始,观察A组进行时空同构模式下的认知功能康复训练,观察B组进行时空同构模式结合心理治疗及认知功能康复训练,对照组则进行常规认知功能康复训练,每天上下午各行一次治疗,30 min/次,共20 d。在训练前、训练后第10天和第20天采用蒙特利尔量表(MoCA)量表、巴氏(Barthel)指数评定表、汉密尔顿(HAMD)量表,对受试者的认知、生活能力、心理状况进行分析,对比三组的干预效果。结果:训练前,三组患者MoCA量表评分、Barthel指数评分、HAMD量表评分比较,差异均无统计学意义(P>0.05);训练后第10、20天,三组患者的MoCA量表评分、Barthel指数评分、HAMD量表评分比较,差异均有统计学意义(P<0.05);训练后第10、20天,观察B组的MoCA量表评分、Barthel指数评分均明显高于对照组和观察A组,而观察B组HAMD量表评分明显低于观察A组和对照组,差异均有统计学意义(P<0.05)。结论:通过这种结合时间、空间关系模拟日常生活环境的新型训练方法,很大程度上满足了患者对空间归属感的需求,激发患者康复训练过程中的兴趣,提高了治疗积极性,有效地改善了患者的认知功能和消极的心理状况,提高了日常生活活动能力,加速了患者的整个康复进程,为临床康复治疗提供了新思路。Objective:To explore the effect of spatiotemporal isomorphism combined with psychotherapy on cognitive rehabilitation of patients with cerebral infarction.Method:From January 2016 to January 2017,120 patients who were first diagnosed as cerebral infarction and had cognitive impairment in the undergraduate department were selected as the research objects.According to the order of admission,the patients were divided into observation A group,observation B group and control group,each with 40 cases.Starting from the 3 rd day after admission,the cognitive function rehabilitation training in spatiotemporal isomorphic mode was observed in observation A group,the spatiotemporal isomorphic pattern combined with psychotherapy and cognitive function rehabilitation training in observation B group,the control group received conventional cognitive function rehabilitation training,each treatment was performed once every day,30 min/time,for a total of 20 days,in the morning and afternoon.The Montreal scale(MoCA),Barthel scale was used to evaluate the Hamilton scale(HAMD) before and after the training 10,20 days,and the psychological status of the cognitive life ability of the subjects was analyzed,and the intervention effects of the three groups were compared.Result:Before training,MoCA scale score,Barthel index score,HAMD scale score were compared among the three groups,the differences were not statistically significant(P>0.05);on day 10 and 20 after the training,the scores of MoCA scale,Barthel index,HAMD scale in the three groups were compared,and the differences were statistically significant(P<0.05);on day 10 and 20 after training,MoCA scale score and Barthel index score of observation B group were significantly higher than those of control group and observation A group,while HAMD scale score of observation B group was significantly lower than those of observation A group and control group,with statistically significant differences(P<0.05).Conclusion:Through the combination of time and space relationship simulated daily liv
