摘　　要：目的比较黏膜下隧道内镜剥离术(STER)治疗消化道黏膜下肿瘤(SMTs)中钩刀与海博刀的手术效果。方法将70例行STER治疗的消化道SMTs患者依据使用器械不同分为钩刀组(n=30)与海博刀组(n=40),比较两组手术指标、住院时间、肿瘤整块切除与完全切除率、并发症及复发情况。结果海博刀组手术时间明显短于钩刀组(P <0.05),器械交换次数明显少于钩刀组(P <0.05);两组凝血钳交换次数、肿瘤切除大小、住院时间、整块切除与完全切除率比较无显著差异(P>0.05);两组3个月内随访均无复发病例;海博刀组并发症总发生率明显低于钩刀组(P <0.05)。结论与钩刀相比,STER治疗消化道SMTs中使用海博刀,可缩短手术时间,降低并发症风险,具有积极应用价值。Objective To compare the effect of hook knife and Haibo knife in the treatment of submucosal tumors(SMTs)in gastrointestinal tract by submucosal tunnel endoscopic resection(STER).Methods A total of 70 patients with SMTs in gastrointestinal tract to undergo STER in our hospital were divided into a hook knife group(n=30)and a Haibo knife group(n=40)according to the different instruments used.The operation indexes,hospitalization time,tumor resection rate,complications and recurrence were compared between the two groups.Results The operation time of the Haibo knife group was significantly shorter than that of hook knife group(P<0.05),and the number of exchange of instruments was significantly less than that of hook knife group(P<0.05);there was no significant difference in the number of exchange of coagulation forceps,the size of tumor resection,hospitalization time,total resection and complete resection rate between the two groups(P>0.05),and there was no recurrence in the follow-up of the two groups within three months;the total incidence of complications in the Haibo knife group(P<0.05).Conclusion Compared with hook knife,the use of Haibo knife in STER for SMTs in gastrointestinal tract can shorten the operation time and reduce the risk of complications;it has positive application value.
