摘　　要：目的探讨术前辅助化疗对胃癌患者手术并发症及生活质量的影响。方法选取2015年2月—2017年2月收治的胃癌90例,按治疗方式分为观察组42例和对照组48例。观察组采用术前辅助化疗联合手术治疗,对照组仅采用手术治疗。对比观察组化疗前后临床分期变化情况,观察2组手术切除情况、相关手术指标及术后并发症发生情况,评价2组术后6个月生活质量。结果观察组化疗后肿瘤分期与治疗前比较明显下降,手术清除率高于对照组,手术时间长于对照组,术中出血量多于对照组(P <0. 05)。观察组术后并发症发生率低于对照组(P <0. 05)。术后6个月,观察组生活质量评分高于对照组(P <0. 05,P <0. 01)。结论对胃癌患者采用术前辅助化疗方案进行治疗,能明显降低患者肿瘤分期,提高手术清除率,减少术后并发症发生率,并能显著提高患者术后生活质量。Objective To investigate effects of presurgical adjuvant chemotherapy on incidence of postoperative complications and quality of life in patients with gastric cancer.Methods A total of 90 patients with gastric cancer admitted during February 2015 and February 2017 were divided into observation group(n=42)and control group(n=48)according to therapeutic methods.Observation group was treated with presurgical adjuvant chemotherapy combined with operation,while control group was treated with operation only.Changes of clinical stages before and after chemotherapy were observed in observation group.In two groups,conditions of surgical resection,related surgical indexes and incidence rates of postoperative complications were observed,and conditions of life quality in 6 months after surgery were evaluated.Results After chemotherapy,tumor staging was significantly decreased compared with that before chemotherapy in observation group.Compared with those in control group,in observation group,clearance rate was significantly higher,and operative time was significantly longer,and intraoperative bleeding volume was more(P<0.05).Incidence rate of postoperative complications in observation group was significantly lower than that in control group(P<0.05).Score of life quality in observation group was significantly higher than that in control group in 6 months after surgery(P<0.05,P<0.01).Conclusion Preoperative adjuvant chemotherapy for patients with gastric cancer may significantly lower tumor staging,improve surgical clearance rate,reduce incidence rate of postoperative complications and significantly improve postoperative life quality.
